1. Download Form 4 by opening the file and clicking on the down arrow icon.
2. Open Form 4B Summary of Ratings of LAC Leaders and enter the required data in the yellow cell. Data entered will be automatically reflected in all 4A LL forms
3. Open Form 4A LAC Leader N / 4A LL N, and enter the remaining required data in the Participant's Profile.
4. Quick Navigation Links can be found in the top-right corner of each individual rating tab. Click on the links to be directed to the tab you require.
5. Open/Get your copy of the LDM2 Evaluation Procedure for Teachers document through this link. Rate Teacher N's LDM2 outputs using the guide questions (Tabl
Ask assistance from the SDO LDM Program Management Team, if necessary. You may also print the rubric for easy reference.
6a. Input the score for each criterion by clicking the down arrow icon in the yellow cell. You may also directly enter the score in the yellow cell. The cell automatically
value has been assigned. You may add qualitative feedback in the Remarks section to substantiate the score/s you have given.
6b. To change the score, click on Delete or Backspace, then do 5a again.
