A RESOLUTION ALLOWING THE SCHOOL TO COLLECT THE AMOUNT OF ONE HUNDRED


PESOS IN SUPPORT FOR THE 26TH FOUNDING ANNIVERSARY OF BAYAWAN NATIONAL HIGH
SCHOOL.

Whereas, Bayawan National High School will celebrate its 26th Founding Anniversary on October 16 – 17, 2019.

Whereas, activities have been lined up in order to make the celebration more relevant and exhilarating. Such activities
are also a culminating activities and presentation of the students’ output in various subjects in their respective
strand/department.

Whereas, the amount of One hundred pesos will be collected from the parents to defray expenses such costume, Mr
Ambassador & Miss Ambassadress 2019, Eco-wear and other expenses relevant thereto.

Whereas, the said contribution is an expression of support by the parents to the school to make the celebration meaning
and successful,

Resolved as it is hereby resolved, proper use of money shall be observed. Report of expenses shall be
presented to the parents during meetings/conferences.

Resolved further, a copy of this resolution shall be forwarded to the Division for proper action.

Now therefore, on motion of Laarni Trayvilla duly seconded by Marlou L. Gayanelo, both Board of Directors
of the PTA that parents should contribute the amount of One Hundred pesos in support for the school as it celebrates its
26th Founding Anniversary..

A RESOLUTION TO OPEN AN ACCOUNT AT BDO UNIBANK, INC – BAYAWAN BRANCH

Whereas, the General Parents-Teachers Association (GPTA) is a recognized parent-teacher association of Bayawan
National High School – Senior High School Department, Villareal, Bayawan City, Negros Oriental.

Whereas, source/s of income are the membership fee/registration fee, contribution, fines and other related
income of the association,

Whereas, for the purpose of safekeeping of the monetary income of the association, there is a need of opening an
account to the nearest reputable bank and all income of the association shall be deposited in this bank.

Resolve as it is hereby resolved, that General Parents-Teachers Associations, GPTA shall open an account
at BDO Unibank, Inc. – Bayawan Branch and shall bear the name, GENERAL PARENTS-TEACHERS ASSOCIATION OF
BAYAWAN NATIONAL HIGH SCHOOL – SENIOR HIGH SCHOOL, (GPTA-BNHSSHS),

Resolved further, that withdrawals of money shall be made only through the school Principal and the duly
elected president and/or treasurer of the association.

Resolved furthermore, that the association shall submit the list of officers or the association’s
organizational set up every year for the purpose of updating and for the change of signatories specifically for
withdrawals of money. The current Principal is DR. CHERRYL MAY A. HONGCUAY, current President is MRS. MA.
MELBA D. FISHER and Treasurer is MRS. LAARNI TRAYVILLA,

Finally resolved, that a copy of this resolution will be forwarded to BDO Unibank, Inc. Bayawan Branch,
National Highway, Bayawan City, Negros Oriental.

Now therefore, on motion of Analyn I. Ongue duly seconded by Marlou L. Gayanelo both Board of
Directors of the association that the General Parents-Teachers Association (GPTA) will open a bank account at
BDO Unibank, Inc. Bayawan Branch, National Highway, Bayawan City, Negros Oriental for the safekeeping of the
monetary income of the association.

Resolved as it is hereby resolved that the Parent Teacher Association of Bayawan National High School
Senior High School Department through its special meeting held on October 15, 2020 at Bayawan National High
School Senior High School Department campus, Villareal Bayawan City Negros Oriental will shoulder the expenses
for the acquisition of the aforementioned equipment subject to usual accounting rules and regulations,
Resolved further that said equipment will serve as donations of parents in support to all the activities of
the school,
Resolved furthermore that a copy of the resolution shall be forwarded to the servicing bank in support
for its withdrawal.
Now therefore on motion of Dianne Kieth B. Arcenal duly seconded by Adelfa A. Dumat-ol both board of
the directors of the association that the said equipment will be acquired to which funds to use will be taken from
the association’s fund subject to usual accounting rules and regulations.
