DHSUD Homeowners Association Certification

This document is a certification form for the registration of a homeowners association. It certifies that the territorial jurisdiction of the association covers either an entire subdivision, specific phases of a subdivision, or an unsubdivided parcel. It also certifies whether any other homeowners associations currently exist within the subdivision and, if so, provides their names. Finally, it identifies the nearest registered homeowners association and provides its location.
