Statement of the Problem
Vaccine hesitancy is described as a delay in accepting, hesitation to accept, or refusal to take
immunization despite the availability of vaccination services. The World Health Organization
(WHO) has listed it as one of the top ten dangers to world health in 2019. Vaccine hesitancy is the
result of a complex decision-making process influenced by a variety of factors.
This study aims to identify the Factors contributing to poor vaccine uptake among the residents of
Barangay __________. Specifically, it sought to answer the following problems:
1. What is the profile of the respondents in terms of their;
a. Age
b. Gender
c. Socioeconomic status
d. Marital status
e. Religion
f. Vaccination experience
2. Does information from vaccinated people, health workers and medical frontliners, social media
platforms and news affects the decision of the residents in getting vaccinated?
3. What vaccination program design can be formulated to increase the uptake of vaccine in
Barangay ___________?

Vaccines acceptance among the general public and health workers appears to have a decisive role
in the successful control of the pandemic. With the availability of COVID-19 vaccines, little
information is available on the public acceptability of Covid-19 vaccine in the city of Urdaneta,
Specifically, in the Barangay of ________________.
This study aims to identify the Acceptability of Covid-19 Vaccine among the residents of Barangay
__________. Specifically, it sought to answer the following problems:
1. What is the profile of the respondents in terms of their;

b. Gender
c. Socioeconomic status
d. Marital status
e. Religion
f. Vaccination experience

2. What is the medical history of the respondents in terms of COVID-19 pandemic?
3. What are the risk perceptions of the respondents towards getting vaccinated?
4. Is there a significant difference between people who are willing to be vaccinated and people who
are hesitant to be vaccinated in terms of the availability of vaccines?
