conduct an online interview with you that will mostly focus on your biographical
information as well as your past health medical history. This Data will be used in order
to provide health care made especially to cater your needs as well as to improve and
possibly solve your health concerns. Please be rest assured that all the listed data and
information will be kept confidential.

Patient: My mother is a housewife, my father is a farmer and my brother and
sister is a high school student

PAST HEALTH HISTORY
So let’s proceed to your Past Health history. I would like to ask if you have any
problems at birth?
Patient: I don't have
You: What are your childhood illnesses?
Patient: I experienced having anemia, Common colds, and flu.
You: did you have any vaccination against flu?
Patient: yes
You: Pollo?
Patient: yes
You: Mumps?
Patient: yes

Patient: at the age of 5 I was hospitalized due to anemia. Now, I'm always
experiencing strong pain in the lower part of my belly, the pain is come and goes
at first, then grows steady and intense. I also lose appetite. It affects me a lot
because I wasn’t able to concentrate.
You: Can you rate its severity from the scale of 1-10, where 10 is the most painful
and 1 is the least.
Patient: I will rate it as 9
You: when did it begin?
Patient: after the day I jogged in the morning
You: How long does it last?
Patient: the pain would last for like 12 hours.
You: do you feel dizzy
Patient: yes, i feel dizzy
You: how about vertigo?
Patient: yes

Patient: yes, my eyes are getting blurry whenever im looking to lights

Patient: yes, sometimes.
You: how about your sense of smell and touch?
Patient: sometimes im sensitive to touch and smell

Patient: yes, apparently.
You: Do you have any trouble sleeping at night?
Patient: yes, almost every night.
You: How much time do you spend on gadgets?
Patient: When I’m attending online class, I always spend at least 8 hours on
Laptop. It depends on our synchronous and asynchronous time. It also included
my time spending on social media. I spend almost my time using my gadgets.
You: Were you involved in any accidents?
Patient: no
You: are you taking any medications? If yes, what are those?
Patient: Whenever I'm having a headache, I take Advil ibuprofen or biogesic.

Patient: none
