0 PURPOSE:
The purpose of this procedure is to provide a systematic process for obtaining work
permit to perform jobs to ensure the safety of personnel and protection of facilities by
means of effective coordination between the working agencies and other functions
involved and to implement appropriate control measures.
2.0 SCOPE:
This procedure shall be applicable to all personnel (employees, contractor’s
employees, and subcontractor’s employee) working at WFB Baird & Company (India)
Pvt Ltd.

3.0 RESPONSIBILITY:
The key responsibility in implementing this procedure will be as follows:

 All personnel working at W.F.B. & Baird (India) Pvt Ltd, Kakkanad site shall adhere
to the requirements of this procedure.

 COO/ Managers/ Safety Officer shall ensure that the procedure is effectively
implemented within the plant.

 Process owners shall issue the work permit after preparing the work area and
ensuring that it is safe to perform the work. Once the work is completed, he shall
check that proper housekeeping is done in work area prior to closing out the
permit.

 Initiator shall implement all safety precautions as per the permit document prior
to commencement of work. He shall also be responsible to ensure that the work
area is handed over in safe condition and with proper housekeeping after
completion of work.

 Area In-charge of the respective areas/Head of Sections that could be affected as a


part of permit activity shall counter sign the work permits as applicable.

 Safety officer shall sign the relevant high-risk procedure as applicable

 COO/Managers/Safety officer shall conduct training programs for Process owners,


Initiator and Gas Testers as applicable and ensure that the Process owners &
Initiators are competent to undertake the role. They shall also conduct regular
audit of work permit process.

4.0 DEFINITIONS & ABBREVIATIONS

Work Permit It is a document authorizing a person to undertake specific work in a


designated area. It serves as an agreement between Process Owners
(Issuing Authority) & Initiators (Executing Authority).

Hot Work It is an authorization document issued to carry out any work that involves
Permit burning, welding, cutting, brazing, soldering, grinding, using fire or spark-
producing tools, open flames or that produces a source of ignition.

Cold Work It is an authorization document issued to carry out any work which is carried


Permit out with no reasonable source of ignition.
Eg: Opening of an equipment, painting, insulation, civil work like flooring
etc.

Work at It is an authorization document issued to work in any place where, if


Height precautions were not taken, a person could fall a distance and liable to
cause personal injury.
It applies when workers are working at heights of 2m or more above a
ground/floor level.

Confined It is an enclosed space large enough for an employee to enter fully and
Space perform assigned work; It is not designed for continuous occupancy by the
employee; Has a limited or restricted means of entry/exit.
Eg:-These space may include underground vaults, chambers, storage tanks,
bins, tankers, tunnels, sewers, boilers pipe lines ,pits, vessels, ventilation
ducts, exhaust ducts, furnaces, diked areas, silos etc.

It is an authorization document issued for entry of personnel into a confined


space.

Confined A permit-required confined space has one or more of these characteristics:


Space Entry ■ Contains or has the potential to contain a hazardous atmosphere; ■
Permit Contains a material with the potential to engulf someone who enters the
space; ■ Has an internal configuration that might cause an entrant to be
trapped or asphyxiated by inwardly converging walls or by a floor that
slopes downward and tapers to a smaller cross section; and/or ■Contains
any other recognized serious safety or health hazards.

Note: This Permit is only for entry in to confined space not for carryout any
job inside. For carrying out job inside, relevant permit shall be taken.

Any man-made cut, cavity, trench or depression in an earth surface formed
by earth removal from a site to form an open face, hole or cavity using
Excavation tools, machinery or explosives

Standard form for approval / clearance by relevant functions for carrying


out excavation/piling in a safe manner.
Excavation
Permit "Lockout/ tagout" refers to specific practices and procedures to safeguard
employees from the unexpected energization or startup of machinery and
Lock out Tag equipment, or the release of hazardous energy during service or
Out (LOTO maintenance activities. This requires, in part, that a designated individual
turns off and disconnects the machinery or equipment from its energy
source(s) before performing service or maintenance and that the authorized
employee(s) either lock or tag the energy-isolating device(s) to prevent the
release of hazardous energy and make sure that the energy has been
isolated effectively.

Lockout devices hold energy-isolation devices in a safe or “off" position.


They provide protection by preventing machines or equipment from
becoming energized because they are positive restraints that no one can
remove without a key or other unlocking mechanism, or through
extraordinary means, such as bolt cutters. Tagout devices, by contrast, are
prominent warning devices that an authorized employee fastens to energy-
isolating devices to warn employees not to reenergize the machine while he
or she services or maintains it. Tagout devices are easier to remove and, by
themselves, provide employees with less protection than do lockout
devices.

Any activity, process or work that has the potential to cause significant
Critical Job personal injury, property damage, upset or trip of the plant.

A High-Risk Procedure is a technique that focuses on job tasks (where key


High Risk tasks are broken down into sub tasks) as a way to identify hazards before
Procedure they occur. It focuses on the relationship between the worker, the task, the
tools, and the work environment. Once uncontrolled hazards are identified,
steps are taken to eliminate or reduce them to an acceptable risk level

5.0 Procedure

5.1 Creation of Work Request


Whenever a maintenance/any other job is required to be carried out, the concerned
Process Owners shall create a work request and send it to the Initiator.

On receiving the work request, the concerned Initiator shall review the nature of
maintenance work to decide whether a work permit is required or not. (Refer
Annexure-2 for works that does not require permit)

5.2 Issue of Work Permit


If the nature of maintenance work does not require a work permit, Process Owner
can start the work directly with information to area in-charge.
If the nature of work requires a work permit, the Initiator shall request the Process
Owners for relevant work permit.
Process Owners shall prepare appropriate work permit (Refer Annexure-3), sign and
issue it after confirming the following measures:

 Equipment/work Area has been prepared and all hazards associated with the
job have been identified, assessed, managed and all relevant parties have
agreed with the safety precautions.

 All necessary supporting documents such as electrical isolation cards /


relevant approvals / High Risk Procedure document/sketches and drawings
are attached to the permit.

 The executing authority knows the exact work location, scope of the work, all
the hazards that are associated with the job and relevant
precautions/controls that must be taken and maintained during execution of
the job.

 Relevant safety preparations, including isolations, de-energizing and de-


pressurizing, Lock-out and Tag-out (LOTO) process have to be complied.

The work permit is made in triplicate (white, yellow & pink form). The Original shall be
issued to the Executing Authority which shall be kept in plastic folder and displayed at the
workplace till completion of work. The yellow copy shall be kept at the respective office &
the pink copy shall be handed over to Safety dept after completion of work.

All other concerned authorities such as Area In-charge, safety Officer, Executing
Authority, Head of other affected department/section shall sign the work permit as
relevant.

5.3 Execution of Work


On receiving the permit, the Executing Authority shall be responsible for
commencing and completing the authorized job while fully complying with the
following requirements:

 Ensure that all concerned authorities have signed the work permit.
 Know and understand the potential hazards involved in the job.
 Know and understand how the hazards are controlled.

 Know the right kind of PPEs required, ensure correct use and compliance
thereof.
 Know the emergencies likely to occur in the job / area and actions to be
taken including evacuating the area.
 Deliver the Tool-Box-Talk to the workers involved, explaining all the hazards
associated with the work and the required safe work practices that need to
be followed prior to the commencement of job.

For all critical jobs, a Job High Risk Procedure (HRP) shall be conducted: List of critical jobs
that need HRP by a team comprising of members from the work area, relevant maintenance
section and safety officer. The HRP shall be attached to the work permit.

The Executing Authority/his team shall carry out the job adhering to applicable
safety precautions. The In-charge of the team/any of the members working on the
job shall display the original permit (white form) near the work place. Managers,
safety officer shall make periodic inspection to ensure the compliance of work
permit requirements.

5.4 Extension of work Permit


If a job accompanied with a work permit is not completed within one Shift (8 hours)
and qualifies for an extension, the Initiator shall obtain extension. If it does not
qualify for extension, a fresh work permit shall be issued by the Process Owner. The
permit can be extended for 15 consecutive days, after that new permit must be
taken. For each shift there must be a process owner & initiator whose signature &
name must be in the permit. The work shall be commenced only after getting valid
permit.

5.5 Closure of work permit


On completion of job, the Initiator shall ensure the site is safe and all tools & tackles,
waste, surplus material, have been removed from the location and isolations have
been reinstated and inform it to the Issuing Authority and Area-In charge.

It shall also be ensured that, all materials that has to go back to store’s inventory are
returned to stores promptly and all equipment that has to be serviced and returned
back to stores are also done.

The Process Owner or designated person shall do a final inspection of the job site to
checks the jobs are completed, proper housekeeping is maintained, scaffoldings are
dismantled (wherever applicable) and ensures that all the permits are returned, all
locks, tags, blinds are removed before energizing and reinstating the isolations. The

work permit shall be signed by both Initiator and Process Owner for closing the
permit.

5.6 Withdrawal of a work Permit


A breach of the permit conditions shall be brought to the attention of the Initiator by
any person witnessing a breach.
On receiving report of a breach, the Initiator shall immediately stop the work until
the situation is discussed with the Process Owner. The Process Owner shall
withdraw the permit if the permit does not adequately address the works being
undertaken and safe practices are not being adhered to.
In addition, a permit may also be withdrawn due to any operational needs of WFB or
on declaration of Emergency at any time.

7.0 ANNEXURES

7.1 Annexure-1 : Types of jobs that requires High Risk Procedure.

Annexure-1- Types of jobs that need High Risk Procedure

These are the following list of jobs for which Job Hazard Analysis (JHA) need to be carried
out before executing the job

1. All height works & critical lifting works.


2. Excavation more than 1m depth
3. Under water works
4. Working on rotating equipment
5. Working on roofing
6. Working on stacks
7. Installation and decommissioning activities
8. Any other job decided by the Process Owners.

Annexure-2: Non-Permit Works

The following list of works does not require work permit.

 Sampling and analysis activities by authorized laboratory staff.


 Normal/Routine works at Workshops, Canteens, and Stores
 Routine gardening activities
 Transportation and driving on main roads
 Solid waste material loading activities
 Floor cleaning and pest control activities in office buildings
 Safety walk/safety tour in plant and other office buildings
 Routine activities at the offices/buildings
 Internal and external audits

Annexure 3: Safety Precautions to be taken for different works

1.0 Preparation of Equipment

1.1 Isolations of equipment/Vessel/System/piping: Ensure the equipment/piping inlet


and outlet valves are completely isolated (disconnection, closing of isolation valves,
double block and bleed valves, provision of blinds etc.) from the rest of the process
to prevent entry of any material, release of energy during the work. Inlets and outlet
valves are to be tagged to prevent unintended opening.

1.2 Depressurizing/Draining of equipment/Vessel/system:


Ensure the equipment is completely depressurized, all materials are drained,
purged/washed with inert gas/steam/water, as applicable, to eliminate flammable
and toxic material from it. Ensure the equipment is ventilated.

Conduct gas test for flammability and toxicity to ensure they are within the safe
limit.

1.3 Depressurizing/Draining of Pipeline: Ensure that the equipment where the pipeline
opening/unplugging work is being carried out is fully depressurized and all chemicals
contents in the equipment/pipelines are drained thoroughly

2.0 Preparation of Work Area:

2.1 Cleaning of foreign Materials


Ensure the area surrounding the work place (within 9-meter radius) is
cleaned from all foreign materials and free from flammable material.

2.2 Information to surrounding


Inform to all adjacent operational areas, which can be affected due to the
work.

2.3 Housekeeping:
Ensure proper house housekeeping is maintained around the work place.

2.4 Provision of Water Hose/Firefighting Equipment: Suitable fire extinguishing


equipment such as pails of water, buckets of sand, hose or portable
extinguishers shall be maintained in a state of readiness to extinguish sparks
and/or possible spillage of chemicals while carrying out the job.

3.0 Barricading
If the work has associated with possible fall of materials, tools & tackles, sparks etc
the work area shall be barricaded appropriately to prevent unauthorized entry in to
the area. For Barricading for work at height, a caution “Be aware that overhead
work under progress and fall of material is likely” shall be displayed. For all other
purpose of Barricading, suitable caution board shall be displayed.

4.0 Tools & Tackles


Ensure that, only certified (Directorate of Factories & Boilers dept, Govt of Kerala
approved competent person) & inspected tools & tackles are used depending upon
the type of work.

5.0 Welding machine safety


While doing welding jobs, ensure the body of the welding machine is grounded and
residual current devices (RCD) are provided. Ensure Isolated trip switches/fuses are
provided which shall trip or blow of the fuses in case of excessive current/short
circuit. Welding cable shall be used without any joints.

6.0 Gas cutting device safety


While gas cutting devices are used, ensure the regulator and hose pipes carrying the
gas are in good conditions and properly clamped at both ends. Check the area
surrounding the gas cylinder for explosive limit to detect and ensure no leakage.

7.0 Fire protection during welding & gas cutting


If there are any combustible material in the vicinity of work place where welding &
cutting jobs are done, those substances shall be removed to a safe place. If they
cannot be removed due to practical difficulties, they shall be covered with
appropriate fire-retardant materials and suitable guards shall also be provided
around the job to confine the heat, sparks, and slag from reaching in to the
combustible material.

Wherever there are floor openings or cracks in the flooring, cracks & holes in the
walls, open doorways and open or broken windows that cannot be closed,
precautions shall be taken so that no readily combustible materials below the floor
or near openings will be exposed to sparks or splinters.

8.0 Electrical Isolation

8.1 Switching off regular power supply Breaker


Disconnect the power supply to the specific equipment by switching off the
mains Breaker of MCC or Breaker at the substation relevant to the particular
equipment.
Reconfirm it by switching on the equipment with field switch. If the
equipment does not start it means, isolation is achieved. If it starts, it means,
relevant breaker is not switched off. In this case identify the appropriate
Breaker relevant to the equipment.

8.2 Switching off Alternative Power Supply Breaker
Ensure the Breaker which supplies alternative power from other plants to the
MCC panel is switched off while carrying Electrical Maintenance job on the
MCC panel. Mention the MCB number and substation number in the permit

8.3 Removal of Fuses


Remove fuses of Breaker if necessary.

9.0 Electrical Locking


Switched off Breaker shall be locked so that it can’t be switched on unintentionally.
Enter the Lock No. in the permit form. Try out is to be done by Process Owners and
witnessed by Initiators.

10.0 Electrical tagging


Switched off Breaker shall be locked so that it can’t be switched on unintentionally.
Enter the Lock No. in the permit form. Try out is to be done by Process Owners and
witnessed by Initiators.

11.0 Mechanical Isolation


The machinery/equipment on which work has to be carried out shall be kept under
shutdown and all energy sources and all isolation points of the equipment shall be
identified. All these energy sources shall be isolated and all stored energy inside the
equipment shall be de-energized. All isolation point shall than be locked-out and
appropriately tagged out.

12.0 Working at Height

12.1 Provision of Ladder


. When Ladder is in use while working at height, ensure that the metal ladder
is not allowed for use in places where overhead power lines are there.
Ensure that the ladder is inspected physically to identify any damage and
deterioration by initiators.
Ensure the person going to work with the ladder understand the concept of a
3-point (two hands and a foot, or two feet and a hand) contact on the ladder
when climbing. Ensure the Ladder is free of any slippery material on the
rungs, steps or feet. Ensure that the maximum load rating of a ladder is never
exceeded.

12.2 Use of Scaffoldings


When scaffoldings are provided for working at height, ensure the scaffoldings
meet the standard with respect to quality of material, footing, load bearing
and stability. Ensure that, life line of 1-inch poly propylene rope is used.
However, in area where there is possibility of heat and flame, use steel wire
instead of poly propylene.

12.3 Work on Fragile Roof
Ensure suitable edge protection is in place prior to access / commencement
of work in fragile roof. Ensure that location below fragile roof surfaces are
suitably cordoned off with barriers and signage.
Ensure a weather forecast been obtained and the weather conditions are
suitable for working safely on a fragile roof.
Ensure there is a safe means of getting persons, material and equipment to
the roof area and back down again. Ensure that, lifeline is provided for
anchoring the double lanyard. Ensure that roof ladder if any used shall be in
good condition.

12.4 Use of Man-basket


When man-baskets are used, ensure that the man-basket is designed by a
professional engineer and constructed in accordance with the approved
design drawings. Ensure the man-basket in use is not expired.
Ensure the rated load capacity is mentioned on the man basket and it is not
used above its rated capacity.

12.5 Use of Guard Rails


When Guardrails are used to prevent worker falling from height, ensure that
Top rails, or equivalent guardrail system members, must be 1m plus or minus
3 inches above the walking or working level. The guardrails must be able to
withstand a force of at least 200 pounds. If there is no wall or parapet, mid
rails or screens shall be provided between the top of the guardrail and the
walking or working surface.

12.6 Use of Safety Net


Ensure Safety nets are provided where there is a potential risk of falling
objects and human beings. Ensure that net is made of rope mesh with
openings not more than 36 square inches. They must be installed as close as
practicable under the walking or working surface on which workers are
working and never more than 30 feet below that level. Nets must be strong
enough to catch a falling worker. The safety net must be a certified one. The
physical condition of the net shall be inspected for wear out damage, or
deterioration before its use by the initiator.

12.7 Use of Safety Harness


Ensure each worker working at height shall wear a body harness connected
by a lifeline to a fixed anchor. The anchor must be able to withstand 5,000
pounds of force, and the lifeline must be made of webbing or have a wire
core.
Ensure workers are trained to use body harness properly. The physical
condition of the harness shall be inspected for damage, or deterioration
before its use.

12.8 Use of Toe boards

Toe boards can be provided when employees below could be exposed to
falling objects. When toe boards are used, they must be erected along the
edges of the overhead walking or working surface for a distance sufficient to
protect workers working below. Toe boards shall be at least 15 cm tall, with
not more than a 1/4" gap at the bottom. Toe boards must be capable of
withstanding a force of at least 50 pounds applied in any downward or
outward direction at any point along the toe boards.

13.0 Confined Space

. 13.1 Gas Test


Ensure gas test is done for presence of toxic/flammable vapors such as
Hydrogen Sulphide, Carbon Monoxide, hydrocarbons and other possible toxic
gases that might likely to be present. The analyzed values to be recorded in
the relevant column of the work permit. Ensure that the detected values are
within than the limits mentioned below.

Carbon Monoxide ≤ 50 ppm, and Hydrogen Sulphide ≤ 10 ppm, and


combustible gas is nil.
The toxic gas shall be checked at a frequency decided by Process Owners and
Safety officer. Ensure the person responsible for gas test is qualified and
competent.
Check for the oxygen content inside the confined space. If the oxygen value
is between 19.5-23.5 %, entry is permitted.

13.2 Removal of Foreign material and Toxic Gases


Ensure all the chemical contents like sludge, corrosive substances are
removed and the confined space is purged with inert media such as
Nitrogen/Steam/Air, to remove flammable and toxic gases.

13.3 Work Environment


Ensure the confined space is cleaned, non-slippery, properly ventilated and
illuminated. Ensure that hand lamps of less than 24 V only to be used for
lighting purpose. Where there is requirement to use higher voltage, double
insulated cable shall be used

13.4 Attendants
A person shall be posted at the manhole of the confined space for helping or
rescuing the persons working inside. He must be aware of the persons inside
the confined space.

13.5 Provision of training


Ensure the entrants, attendants, supervisors, watch have attained confined
space entry training adequately.

13.6 Provision of suitable access

Ensure provision of suitable entry and escape to the confined space by
suitable means using appropriate ladder/scaffolding.

14.0 Excavation
14.1 Soil Test/study
Ensure a soil study/test is done to understand the characteristic of soil. Make
trial pit wherever necessary

14.2 Underground Installation


Ensure an accurate up to date drawing(s) of the site showing the location of
all the services in the area, and the boundaries of working area defined.
Confirm with concerned head of Engineering, Head of IT, Head of Operations
to confirm (as per the drawing) if there are any underground cables or
pipelines, such as telephone lines, control cables, sewer lines, fuel lines,
water lines or any other underground installations that could reasonably be
expected to be encountered during excavation work.

Where the existing drawing can’t confirm/doubtful about the underground


installations cable detector shall be used to confirm it.
In case of existence of underground installations, ensure that special
protection is taken to avoid damage them

14.3 Barricade for excavation


Ensure that the open sides of excavation are properly barricaded to avoid
unauthorized entry of people and risk of falling inside the excavation.
Ensure no excavation jobs are done at dark hours, unfinished excavation site
shall be provisioned with danger light so that it is visible during dark hour.
Ensure that any excavation pit more than 3 feet shall be hard barricaded.

15 Radiography

15.1 Radiographer Eligibility


The radiographer shall have authorized & Valid (TLD Badge)
certificate with him. The radiographer shall be physically fit for the
work and submit a medical certificate from a qualified medical
practitioner.

15.2 Control Area


Ensure that radiography is done in an area designated as a controlled
area where specific protection measures and safety provisions are in
place. Ensure the boundary of the controlled area is set at a dose rate
which is appropriate under the prevailing circumstances and is
authorized by the Regulatory Authority.
Ensure the boundary of the controlled area is demarcated by
appropriate physical means such as barricading, flickering light etc.

15.3 Warning notices/signal
Ensure that, notices are displayed at the controlled area boundary at
suitable positions. The notices shall bear warnings and appropriate
instructions.
Ensure Visible or audible signals are used where a radiographic source
is exposed.

15.4 Patrolling and monitoring the boundary


Before the start of radiographic work, Ensure the area is cleared of all
people except for authorized personnel. The boundary shall be clearly
visible and well-lit and continuously patrolled to ensure that
unauthorized people do not enter the controlled area.

Ensure the dose rates at representative points at the boundary are


checked during radiography.

15.5 Shielding
Ensure appropriate shielding so that the radiation beam is primarily in
the direction necessary for radiography and reduces the radiation
doses received by radiographers.

15.6 Monitoring
Ensure that suitable monitoring devices are made available to the
radiographers working with ionizing radiation. These devices shall be
regularly assessed for the radiation, to which they have been
exposed.

16.0 Crane Lifting


16.1 Licenses/Fitness Certificates/inspection for crane
Ensure the annual crane load test certificate, certificates for wire
ropes, Slings, D-shackles and other lifting accessories issued by a
competent authority. Ensure availability of RTA-registration valid
certificate.
Ensure the slings and shackles have valid test certificate from the
Directorate of Factories & Boilers approved competent person and
are visually inspected for defects and damages by the
initiators/executing authority.

16.2 Obstacles
Ensure that there are no overhead obstacles such as power lines,
radio frequency towers near the crane lifting. Ensure that there are
no nearby buildings, equipment, structures, stacked materials that
may obstruct the lifting operations from being carried out safely.

16.3 Environmental conditions


Ensure that the lifting operations shall not be preceded if there are
thunderstorms, lightning strikes and strong wind blows more than 20
mph.

16.4 Site Condition


Ensure that the crane is operated only on uniform, level and firm
ground with sufficient load bearing capacity, which can withstand
maximum in-service load of the crane. Ensure that adequate space is
available for crane standing, access, erecting, and operations and
dismantling. Ensure that the cranes standing in the place are clear of
all underground services. If it is not possible, the services shall be
protected adequately to prevent damages.

16.5 Crane Cabin


Ensure that the cabin of the crane shall have a manufactures manual
containing all pertinent data related to the safe operations of the
crane. A log book shall be maintained in the crane cabin for recording
of all tests, inspections, repairs details.
The cabin shall be provisioned with a roof of adequate strength to
protect operator from falling objects, it shall be properly ventilated
and have a safe access to and egress from the cabin.

16.6 Crane Load capacity


Ensure the crane has a durable safe working load chart to adhere to
the following:
For crane with load indication device and having over load cut-off
facility, ensure the total operating load is less than 90% of the
allowable crane load capacity.
For crane without load indication device and which does not have
over load cut-off facility, ensure the total operating load is less than
80 % of the allowable crane load capacity.

16.7 Licenses/Fitness Certificates for crane Operators


Ensure availability of valid driving license issued by RTA for the crane
operator.

17.0 Fire Water Closure

Process Owners shall notify to Chief Emergency Controller, Chief Incident Controller,
and all Sections and Facilities who get affected non availability of Fire water System.
Process Owners shall ensure Readiness of fire extinguishers to deal with Emergency
Situation during the maintenance work of Fire water system. Initiator shall asses the
work and inform the time period for the job and this information shall be recorded
in the work permit.

18.0 Chemical handling


Ensure that all Hazards pertaining to RLNG, Hydrogen peroxide, Caustic, acids, dyes
etc & handling of the chemicals are known to the work men. Ensure that the safe
storage practices and safe disposal methods mentioned in the Safety Data Sheet
(SDS)/chemical suppliers are implemented.
Ensure any non-compatible chemicals are not stored along with special chemical.

19.0 Personnel Protective Equipment (PPE)
