4. I will scrupulously follow the instructions given by Supervisor and the Invigilator at all the stages of the examination. If
Examiners/Invigilators found me either copying or receiving assistance from others or offering assistance to others and or using mobile
phone etc., i will be disqualified.

● Copy of Printed filled form along with receipt of Rs.1500/- for each program as University Admission Test fee deposited in AC#
148700074507-01, shall be submitted to DASAR office UAP, Peshawar.
● Without Original Bank deposit Slip your application form will not be entertained.
● Mobile Phones are not allowed in Test Center premises.
● Last date for submission of application form is Monday 12 October 2015.
● Application should reach DASAR office latest by last date of submission of Application Form.
● You can download your Roll number slip online from [Link] from 16 October 2015.
