You must provide a certified copy of the legal name change document, such as a marriage certificate, divorce decree, or a court-ordered name change document. This document will be returned in a separate mailing from your new passport .
The check or money order must be made payable to the 'U.S. Department of State', and it is crucial to print the full name and date of birth of the applicant on the front. This ensures that the payment is correctly associated with the applicant's passport renewal application, helping to avoid processing errors and delays .
The applicant may request a larger passport book with 52 pages at no additional cost by checking the '52 page' box at the top of the DS-82 form .
For expedited processing, there is an additional fee of $60 on top of the regular application and execution fees. For example, the passport book fee would total $205 ($110 + $35 + $60), the passport card $125 ($30 + $35 + $60), and the passport book & card $235 ($140 + $35 + $60).
The photo must be color, taken in the last 6 months, with a clear image of the face. Filters commonly used on social media should not be used, and someone else should take the photo—selfies are not allowed. The applicant must take off eyeglasses for the photo, and it should have a plain white or off-white background .
Social media filters are discouraged as they can alter the natural appearance and likeness required for identification purposes. These requirements ensure the photo is a true representation of the applicant, which is essential for security and identification checks at airports and borders .
If you do not have a Social Security number, you must submit a signed and dated statement including the phrase, “I declare under penalty of perjury under the laws of the United States of America that the following is true and correct: I have never been issued a Social Security Number by the Social Security Administration” .
You must submit your most recent passport with your renewal application. The old passport book or card will generally be returned to you in a separate mailing from your new passport .
The photo should be stapled to the application using 4 staples vertically in the corners as close to the outer edges as possible .
Since October 29, 2020, U.S. citizens born in Jerusalem can request either 'Jerusalem' or 'Israel' as their place of birth when applying for a U.S. passport. However, you must pay all applicable fees if you choose to renew or reapply for a passport to change your place of birth to 'Israel'. If not specified, the place of birth will be indicated as 'Jerusalem' .
