LESTER LAVALAIS,
Your claim for unemployment benefits was filed on October 19, 2021. Your monetary eligibility, as well
as important information on returning to work and filing bi-weekly certifications, is explained below.
Read this letter carefully and keep it for reference during your claim; you will be held accountable for
the information. Your claim for unemployment benefits is denied because you did not earn enough
wages during your base period. You do not meet the monetary requirements to establish a claim.

Qualifying for a Claim
In order to monetarily qualify for benefits you must meet the wage requirements shown below. The
wages in the first four of the last five completed calendar quarters are used as the base period.
• You must have earnings in at least two quarters in the base period.
• Your wages must be at least $3,744.00 in the quarter you had the highest earnings.
• The total wages of the entire four quarters of your base period must be at least 1.5 times the gross
wages paid in your highest quarter.
Or
• You must have earnings in at least two quarters in the base period.
• Your total wages equal 20 times the state average weekly wage. ($21,666.60 for 2021)
If you do not qualify when using the first four quarters as a base period, the last four quarters of the last
five completed calendar quarters are used as the base period. The same requirements are then used
with this different set of quarters.
You must also meet other requirements to be eligible for benefits. For example: If you quit or were
fired from your job, Unemployment Insurance Agency will request information from you and your
employer about your separation. A determination will be made regarding your eligibility for benefits
and this will be mailed separately.
Steps to Re-employment
Your obligation while receiving unemployment benefits is to become re-employed. Your responsibilities

for this plan include two steps.
Step One: Register for Work and Visit Your Local Michigan Works! Service Center
You will also need to visit your local Michigan Works! service center in-person to verify your Pure
Michigan Talent Connect profile. While there, you can create or update your resume, sharpen your
interview skills and more. Go to [Link] or call 1-800-285-WORK (9675) for the
service center nearest you. You are encouraged to take advantage of the resources available at your
local Michigan Works! Agency Service Center. It will make a positive difference for your job search!
Visit Pure Michigan Talent Connect at [Link] to create a profile. You may also use this
website to search thousands of jobs, receive email updates about new jobs, and connect to valuable
programs and tools.
These activities must be done three days prior to your first certification for benefits. Failure to register
for work and visit a local service center may prevent the payment of your unemployment benefits.
Step Two: Actively Search for Work Every Week
A primary component of your re-employment plan is to seek work with employers. This includes
making at least two job contacts every week.
Keep a record of these activities, including (1) Date of Contact; (2) Name of employer; (3) Physical or
online location of each employer; and (4) Method by which work was sought. You can submit these
details when filing your bi-weekly certifications online or by submitting Form UIA 1583, Record of Work
[Link] activities must be reported every month. These activities can be reported more
frequently when filing your bi-weekly certifications online. Your work search is subject to audit and
verification. Failure to conduct an active work search and report the details of this activity may prevent
the payment of your unemployment benefits.
Bi-Weekly Certification
You must certify your eligibility every two weeks to receive payment. To certify, log into your Michigan Web
Account Manager (MiWAM) at [Link]/uia. Your first certification must be done anytime between
October 31, 2021 and November 06, 2021 and then every two weeks thereafter.
After your first certification, you must continue to certify bi-weekly. You can certify in your online account seven
days a week and 24 hours a day. If you do not have a MiWAM account, you can create one online at
[Link]/uia. If your claim has been denied, you should continue to certify while your protest is
pending; otherwise, if the protest is resolved in your favor you cannot be paid for any weeks that you did not
certify.
Bi-Weekly Certification Day and Time
You can also certify by phone by calling MARVIN (Michigan Automated Response Voice Interactive Network) at
1-866-638-3993. If you certify using MARVIN, you must certify on your MARVIN scheduled certification
day and time.
If certifying by phone, your filing day and time are found by using the last two digits of your Social Security
Number. If you are unable to certify on your scheduled day and time, you may certify on either Thursday or
Friday which are make-up days.

Example: If your Social Security Number is 555-55-5511, the last two digits are 11. Number 11 is under
Monday.
Inform UIA of Your Work Search Activities
You are required to actively seek work and report at least one weekly work search activity during your bi-weekly
certification for benefits. Your benefits will not be paid until you report your work search activities to UIA.
Report your work search activities during your bi-weekly certification online through your Michigan Web Account
Manager (MiWAM) account which is the preferred and quickest method.
You may also report your work search when you certify with MARVIN by calling 1-866-638-3993. After
completing the automated questions, you must stay on the line to be connected with an UIA agent and
provide your work search information for each week.

Preserving Benefit Entitlement
If you were disabled due to an involuntary disability lasting more than 14 consecutive days within the
past three years, and your claim for benefits is denied for insufficient wages or no wages in the past 18
months, call UIA at 1-866-500-0017 regarding the possibility of preservation of benefit entitlement. The
request for preservation of benefit entitlement must be submitted within 90 days after your disability
began. If your medical condition prevents you from submitting this request on time, you must submit
your written request within 90 days after your period of inability to file has ended. You can also
preserve your benefit entitlement if you submit your request within 90 days after being advised by the
Agency of your right to file for preservation of benefit entitlement. However, in any event, your request
must be made within three years after the disability began. Be prepared to furnish the Agency with a
statement from your physician.
If you disagree with the wages reported by your employer(s), or if the employer did not report wages for
a quarter in which you worked, you must complete Form UIA 1718, Unemployed Worker's Statement of
Wages and return it, along with proof of earnings (W-2, check stubs, employer print out, etc.) to the

address listed on the form.
Penalties
It is against state law to intentionally make false statements or conceal material information to gain or avoid the
payment of benefits. You may have to repay up to 1.5 times the amount of benefits received. Benefits will be
stopped, and any remaining benefits will be lost. You may also be subject to criminal prosecution. If prosecuted,
you may be required to pay court costs and fines, face jail time, perform community service or any combination of
these.
Protest Rights
If you disagree with this determination you have the right to protest requesting a redetermination. Your protest
must be received within 30 calendar days from the mail date of this redetermination. If your protest is filed after
the deadline, you must include the reason your protest is late in your statement. You can also attach copies of
any documents that support your protest.
You can submit your protest online at [Link]/uia through your Michigan Web Account Manager
(MiWAM) and upload documents. If you wish to protest in writing, complete Form UIA 1733, Protest of a
Determination. This form is located on the website under the Forms link. Include your name, case number and
social security number or Michigan Identification Number (MIN) on documents submitted with your protest. Fax
the completed Form UIA 1733 and any supporting documents to 1-517-636-0427 or mail to Unemployment
Insurance Agency, P.O. Box 169, Grand Rapids, MI 49501-0169. If you fax or mail your protest, it must be
signed.
Your protest must be received no later than November 19, 2021 or this determination becomes final.
Preferred Method of Correspondence
Immediately notify us of any changes to your address and/or telephone number. This is an important part of being
available for work. You can update your method of correspondence, address, or telephone number using your
MiWAM account.

IN PERSON: Visit your local Unemployment Insurance Problem Resolution Office (PRO) Monday through Friday,
8:00 AM until 4:00 PM Eastern Time. To find the nearest PRO, go to the website under UIA Quick Links for a list
of locations.
IN WRITING: Fax correspondence to 1-517-636-0427 or mail to Unemployment Insurance Agency, P.O. Box
169, Grand Rapids, MI 49501-0169. Include the date, your name, Social Security number, and signature on all
correspondence. Retain a copy for yourself. Print and keep the confirmation that your Fax was received.

and number of weeks allowed.
Your information, including but not limited to Social Security Number and Driver’s License Number, may be
verified through computer matching programs.
If there is a question regarding the identity of the claim filer, UIA may conduct fact finding by contacting the filer at
the number provided on the claim. If there is no immediate response, the claimant will have 48 hours to return the
call. If no response is received, a determination regarding identity will be made based on the information
available.
