Dear Sir/Ma’am,

I am ANALOR D. CANTOR, mother of Czzar Dumaliang Cantor who is a 2nd year BS Marine
Engineering student in this beloved institution. I am writing to request for a copy of my son’s
Certificate of Grades for the first and second semester of the school year 2020 – 2021 for the
processing of his scholarship from OWWA.

Unfortunately, I am currently working abroad and my son lives in Isabela so I am unable to


request from your good office personally. In this regard, I would like to authorize my good
friend MRS. EMMA A. PEREZ to request and claim the above-mentioned document on my
behalf.

I attached in this letter a scanned copy of my ID as proof of my identification for your


reference.
