• Observing:
Is to gather data by watching
carefully the way something
happens or the way someone
do something, especially in
order to learn more about it.

• Interviewing:
A conversation in which
someone is asked questiobs
about themselves or a subject
they know about.

• Examining:
Is a systematic data collection
method that uses observation
(i.e senses of sight, hearing, smell
and touch) to detect health
problems.
DATA COLLECTION
• Subjective Data – It is referred to as
symptoms or covert data

• Objective Data – It is referred to as


signs or overt data detectable by
an observer

1. Family structure, characteristics and dynamics

• Include composition and demographic data of the


members of the family/ household, their relationship
to the head and place of residence; type of,and
family interaction/communication and decision-
making patterns and dynamics.

2. Socio- economic and cultural characteristics

• Include occupation, place of work, and income of


each working member; educational attainment of
each family member; ethnic background and
religious affiliation; significant others and the other
role(s) they play in the family’s life; and, the
relationship of the family to the larger community.

3. Home and environment

• Include information on housing and sanitation


facilities; kind of neighborhood and availability of
social, health, communication and transporation
facilities in the community.

4. Health status of each member

• Includes current and past significant illness; beliefs


and practices conducive to health and illness;
nutritional and developmental status; physical
assessment findings and significant results of
laboratory/ diagnostic tests/ screening procedures.

5. Values and practices on health promotion/


maintenance and disease prevention

• Include use of preventive services; adequacy of


rest/ sleep, exercise, relaxation activities, stress
management or other healthy lifestyle activities,
and immunization status of at-risk family members.
Individual Data Base

This tool is use to identify existing and potential


wellness state/s, health threats, health deficits, and
stress points/ foreseeable crises in a given family.
