● What I Know: Police Blotter is a complaint of an individual, this is the immediate process
done when filing a report / complaint
● What I Learned: It is a written record of arrests and other occurrences maintained by the
police. The report kept by the police when a suspect is booked, which involves the
written recording of facts about the person’s arrest and the changes against him or her

● What I Know: Affidavit of arrest is a report when an arrest was conducted
● What I Learned: it is generally filled out by the arresting officer and states the fact and
circumstances surrounding an arrest. The affidavit may state such facts as the
information which led to the arrest and the observations made before and after the arrest
What is the Difference between Affidavit of Arrest from affidavit of complaint?
● What I Know: the difference of an affidavit of arrest from complaint is that the arrest is
done when an arrest was made while affidavit of complaint is a statement where a
complainant is complaining someone
● What I Learned: An affidavit of complaint is a sworn statement prepared by someone
who wishes to file a legal complaint while Affidavit of arrest is generally filled out by the
arresting officer and states the facts and circumstances surrounding an arrest

AFFIDAVIT OF COMPLAINT
I, Bark Crosini, of legal age, married, Filipino and a resident of #2San Antonio, Sta Cruz
Laguna, after having been duly sworn to according to law hereby depose and say that:
1. On December 01, 2014, at around 6 o’clock in the evening, before leaving my
establishment, Trinity Appliance Center, I entrusted the key to my salesman
Bose Femme, with confidence asking him to lock and secure the Appliances in
the establishment.
2. At around 8 o’clock that evening, I called Bose femme to make sure he has
already locked the establishment but he did not answer his phone.
3. On December 02, 2014, at around 7 o’clock in the morning, I went to the
establishment and was surprised because my salesman Bose Femme had not
yet arrived.
4. When I was about to call him, the saleslady of the establishment nearby passed
by so I asked her if she had seen Bose Femme. She said she had not see him
that morning but had seen him last night. He was carrying a back pack and he
was rushing. Instead of a passenger jeepney, he rode a taxi saying “MAUNA NA
AKO, DADALHIN KO PA KASI ANG MGA ITO SA MGA UMORDER” (Affidavit of
witness, Ms. Anna Dimaculangan is hereto attached as “ANNEX A”
5. After conversing with the saleslady, using the duplicate key, I opened my
establishment. I noticed that valuable items were missing three Rice Cooker
with serial numbers 502108, 553019 and 501879 respectively, all valued at
P7,500.00, two Single Induction Cooker with serial number 782134 and
784015 all valued at P35,000.00 and two Single Gas stove all valued at
P3,500.00 with total amount of P46,000.00. Philippine currency
