Weekly Excavation Inspection Checklist

This document is an excavation checklist used weekly to inspect excavation sites and ensure safety protocols are followed. It was prepared by Andrew Merrison, Group Head HSE and authorized by Adnan Naji, Chief Operating Officer of Saif Bin Darwish. The checklist contains 16 items to inspect, including confirming the excavation permit is obtained and displayed, underground services have been marked, protective systems like shoring and sloping are adequate, access points and stockpiled materials are properly placed, and required PPE is being used. 1. Has the excavation permit been obtained, correctly
completed, and displayed at site?
2. Have the under ground services been detected
and marked prior to excavation?
3. Does the permit specify any special hazards and
appropriate controls thereof?
3a. If YES in No.3; Are the controls known and being
implemented by the workers?
4. Is the method of excavation, stated in the permit,
appropriate and being followed?
5. Is the excavation shored, sloped, or shielded
adequately during all stages?
6. Are there signs of water seepage? If so, is a de-
watering system in place?
7. Is there any damage to the excavation protective
system (shoring/sloping/bench)
8. Is there any visible damage, cracking, collapse or
other signs of weakening?
9. Are there access/egress points, a max. of every
8m, for excavation over 1.2m deep?
10. Are stockpiled materials situated at least 1.5m
away from the edge of the excavation?
11. Have stop blocks been placed for vehicles
reversing back onto the excavation?
12. Have adequate safety signs, warning tape, lights,
and barricades been placed?
13. Are sufficient controls in place for pedestrian
and/or vehicle crossing points?
14. Have toolbox talks been conducted and recorded
to explain the hazards to workers?
15. Are there enough controls to prevent vibration from
vehicles passing nearby?
16. Is the PPE, stated in the Excavation Permit being
used by all correctly?
