AFFIDAVIT OF LOSS

I, _______________, of legal age, Filipino citizen and a resident


of ___________________, after being duly sworn to in accordance with
law hereby depose and say that:

1. I an employee of the Municipality of ____________ as Engineering


Assistant;

2. I was required to attend a seminar at _____________, as such I


secured a cash advance for my travelling expenses;

3. A check was issued in my name for the said purpose.

4. The said municipality issued a check dated August 30, 2018


bearing the No. 1150646 and containing the amount of Two
Thousand Five Hundred Eighty pesos (2,580) for the said purpose;

5. I have placed the said check in my folder but when I needed it for
liquidation of the municipality, I found out that it was already
missing;

6. I tried my best to locate the same but my efforts were in vain;

7. I consider the said check as lost for all legal intent and purposes;

8. I attest to the truth of all the foregoing facts.
