The teacher lets each group formulate 5 possible questions that are answerable
with yes/no in the given problems to the group.

4. Tardiness
5. Lack of interest

Abstraction 10 Minutes The teacher asks the questions:


1. What is a survey questionnaire?
2. What are some of the questionnaire designs used in your survey
questionnaire?

The teacher processes the students answer and discuss about a survey
questionnaire. The teacher uses the questionnaire used by the students as one
of the example. The following topic
1. The Designs of questionnaire
2. Ethics of a good interview questions; and etc

Application20 minutes Group Activity: Writing Survey Questionnaire


Group the learners into five (5) and assign the following topic to design Survey
Questionnaire on:
1. The best Commercial ads seen on Philippine television
2. Favorite Teacher of the Month
3. The most listened FM radio station
4. The most brand of ball pen used in school
5. In demand snacks in the canteen

*(Let the students decide their own topic.)
