INTRODUCTION:
The purpose of this paper is to explore and identify those elements which make the provision of
health care services inaccessible to the patients. After the detailed analysis of that issue, the paper
presents the series of solutions that can mitigate the accessibility of health care services and by
evaluating the pros and cons of each solution the paper finally presents home health care services as
the most effective, efficient, and helpful means of increasing the accessibility of the health care
services to a vulnerable segment of society.

ELEMENTS OF PROBLEM
1) Geographic area:
Patients can not have access to the health care services if it is not available in their geographic area.
Mostly such areas are rural which have a dearth of doctors and health facilities. [ CITATION
Nat18 \l 2057 ] according to the study conducted in the rural areas of the USA reveal that Rural
inhabitants were observed to have weaker health than urban dwellers, and rural regions had more
trouble in enticing and holding doctors than their urban equivalents [ CITATION Dou15 \l 2057 ]

2) TRANSPORT:

If the service is available in the area then accessibility of the health care services might face some
other obstacles such as insufficient transportation facilities. There can be multiple reasons for issues
of transportation such as travel period is too much, no community transport is accessible,
unavailability of personal car or bike or expensive transportation. [ CITATION Nat18 \l 2057 ]

3) DISABILITY :

Another issue is the disability due to which people cannot get access to health care and therapy
services. Most disabled people are located in the region of Sub-Saharan Africa who apart from
disability face abject poverty, low literacy rates, and poorly qualified healthcare specialists.

ANALYSIS OF PROBLEM
The above mention problems of geographic areas, transport, and disability are discussed and
declared by many other studies as a cause of inaccessibility of the health care services. in rural
areas. Such as the [ CITATION Flo18 \l 1033 ]research reveals that Peruvian aged people living in
severe deprivation have restricted access to health care facilities. Moreover, [ CITATION Sye13 \l

1
2

2057 ] declares that Transportation impediments cause postponed or lost schedules, suspended care,
and skipped prescription usage. These outcomes may lead to weaker supervision of persistent
sickness and thus worse health consequences. furthermore, [ CITATION Gra12 \l 2057 ] discusses
that the rural population faces difficulty in acquiring health care services due to the unavailability of
doctors especially the specialists nearby. This increase considerable burden due to excessive travel
period, expense, and time away from the office.

CONSIDERING OPTIONS:
1) Online health service information
The great quantity of health material accessible online offers clients with better access to data
pertinent to the administration of health situations. This is mainly significant given a snowballing
motivation for consumer-focused health maintenance schemes universally, particularly in the
managing of prolonged health situations, and in acknowledgment of problems confronted by
unprofessional consumers with the outcome, understanding, and performing on health data obtained
online.[ CITATION Lee14 \l 1033 ]

2) TELEHEALTH
The telehealth according to WHO is the supply of health care facilities, where distance is a life-
threatening Feature, by all health care specialists using data and Communication machinery for the
interchange of effective material for analysis, treatment, and avoidance of illness and wounds,
investigation, and assessment, and for the ongoing schooling of health care workers, all in the
welfares of progressing the health of People and their societies [ CITATION Pra15 \l 1033 ]
According to the research of the National Broadband Plan the usage of distant patient nursing
expertise in combination with automated health registers can save up to $700 billion for 15 to 25
years of health care business.

3) HOME HEALTH CARE :

Home health care is a broad array of health care facilities that can be provided at a patient home for
sickness or wound. Home health care is typically less costly, more accessible, and just as useful as
treatment patient receives in a hospital or at a professional nursing service. Home health care will
decrease the burden on patient family members who jungles between jobs and take care of their
parents.
Individual care employees and home care assistants offer more private care support to sick people in
the home arrangement. Their duties primarily emphasize activities of every day residing (e.g.,
cleansing, grooming, clothing, eating), housework, etc. moreover, Home health care is the quickest

rising segment in the health care business, with projected progress of sixty-six percent over the
coming decade 10 years. It assists almost seven million sick people annually (Gershon, et al., 2008)

FINAL SOLUTION:
Usually, the poor population in rural areas or disable vulnerable people lack access to the internet
and they are uneducated s well which will make it difficult for them to use online health
information. Similarly, the lack of accessibility to phone will create hindrance in telehealth.
Moreover, in telehealth doctors can not diagnose and treat properly. Neither doctor can send
medicines. Hence, the best solution to increase the accessibility of health care is home healthcare
services. Because the implementation of home health care services will overcome the hindrance pf
low education, poverty, disability and transportation as a doctor and medical help will reach home
with proper medicine and examination kit which will not only be cost-effective of that poor
population but will also help to reduce disease from such vulnerable group of societies.

ETHICAL IMPLICATIONS :
The provision of home health services is ethical because this provision of this service is feasible in
every aspect. Through this, not only the vulnerable section of the society will get access to health
care services but it will also help to divide the financial and other resource burden bore by the
hospitals. These services will reduce the inequality from the society by giving equal opportunity to
access health care facilities to marginalized society.

IMPLEMENTATION OF SOLUTIONS
Home health care services need to be designed at the state level and synchronized as packs of care
that are personalized and provided to patients in their residences. It should be inexpensive by the
society and offer satisfactory excellence of treatment. It is also the greatest inexpensive means to
improve accessibility to key health care facilities for such a helpless group[CITATION The15 \l
1033 ]

CONCLUSION
Hence, the research reveals that problems in the accessibility of the health care services are the
geographic areas that lack transportation services and basic health facilities. Moreover, poor people
cannot afford health services. Along with this disabled can not get medical help as it is difficult for
them to visit doctors. Thus to overcome this issue online medical information, Telehealth and health
care services can be used to reduce this gap. However, the most reliable, cost-effective, and useful

solution is home health care services which will be accessible to everyone who cannot visit a doctor
due to any cause.

Potential solutions to overcome transportation and geographical barriers include implementing telehealth services and developing community transport programs . Telehealth facilitates remote access to medical consultations, reducing the need for travel and increasing timely access to care . However, it requires reliable internet access, which is not always available in rural areas . Community transport programs can provide scheduled transport services for patients to healthcare facilities, addressing the lack of personal or public transportation . These programs, however, require significant investment in logistics and resources to maintain efficiency and reliability.

Home health care services can significantly reduce healthcare costs and ease resource allocation in hospitals by providing medical care at the patient's home, thereby decreasing the need for hospital admissions . This reduction in hospital visits alleviates the financial strain on healthcare facilities and ensures hospital resources, such as beds and staff, are available for patients with more acute needs . Home healthcare shifts the focus from expensive hospital-based care to more cost-effective community-based care, potentially reallocating resources towards preventive and personalized care initiatives .

Telehealth improves access to healthcare services by allowing patients to receive medical consultation and advice remotely, which is crucial where distance poses a significant barrier . This method can save substantial healthcare costs and allows continuous education of healthcare workers, enhancing overall community health . However, its limitations in rural and underserved communities include issues related to technology access, as not all members of these communities have reliable internet access or the necessary devices to utilize telehealth effectively. Moreover, telehealth may not allow accurate diagnosis or treatment, and doctors cannot prescribe medications through this platform .

Home healthcare plays a vital role in enhancing healthcare accessibility for vulnerable populations such as the elderly, disabled, and those living in rural areas. It offers a wide range of health services directly to the patient’s home, reducing the need for travel and providing personalized care . Home healthcare is less costly than hospital care and helps reduce the burden on family members . It effectively addresses issues of low literacy, poverty, and transportation, making it the most reliable and cost-effective solution for increasing healthcare accessibility, as it provides necessary medical services and medication administration at the patient’s home .

Implementing home healthcare services as a universal solution is feasible in that it reduces hospital resource burdens and provides equitable healthcare access to marginalized communities . Ethically, it represents a fair distribution of healthcare resources, potentially reducing societal health inequalities by offering equal access opportunities . However, challenges include required state-level coordination, ensuring affordable care, and maintaining high treatment standards in the home setting . Ensuring these aspects implies careful planning and integration within existing healthcare frameworks to achieve ethical and practical success.

Overcoming healthcare access barriers in rural communities can improve overall public health by facilitating early diagnosis and better management of chronic conditions, leading to reduced disease progression and complications . Improved access to healthcare also promotes preventive care, reducing the incidence of preventable diseases and associated healthcare costs . Economically, reducing health disparities allows individuals to maintain productivity and contribute to local economies, enhancing workforce stability and community development. Increased healthcare access can attract more healthcare providers, creating jobs and stimulating economic growth in rural areas .

Transportation barriers significantly affect healthcare access and outcomes in rural regions by causing delays and missed medical appointments, ultimately leading to poorer chronic disease management and health outcomes . The absence of reliable transport options or excessive travel times and costs deters patients from seeking timely care, resulting in advanced progression of illnesses that could have been managed better with regular medical oversight . These transportation challenges compound the existing lack of healthcare facilities and professionals, further exacerbating health disparities in rural areas .

In rural areas, the primary geographical and logistical barriers to accessing health care include a lack of available medical facilities and a shortage of healthcare professionals. Rural inhabitants often exhibit weaker health compared to urban dwellers, partly because rural regions struggle to recruit and retain doctors . Additionally, issues like insufficient transport facilities worsen accessibility, as patients may face prolonged travel distances, lack of community transport, or the high cost of personal or public transportation . These barriers contribute to delayed or missed medical appointments, poor chronic disease management, and consequently, worse health outcomes .

Online healthcare information services support patients by providing easy access to a vast range of medical data and advice crucial for managing chronic illnesses. This access empowers patients, enabling them to take a proactive role in their health management . However, challenges include the digital divide, as not all patients, especially those in rural or poorer areas, have internet access or skills to navigate online health data effectively. Moreover, some patients find it difficult to understand or act on this information due to its complexity or lack of professional guidance .

To implement home healthcare services effectively on a state level, strategies should include developing state-coordinated care packages that are personalized to patient needs, ensuring affordability and high-quality care . Coordination with existing healthcare providers is crucial to integrate home healthcare into broader health systems smoothly. Strategic planning should focus on training home healthcare workers and establishing robust logistical support for delivering medical supplies. Additionally, legislation should support home healthcare initiatives through adequate funding and policy frameworks that encourage community-based solutions and sustainable service models .
