Important Note:
1. This registration form serves as your proof of enrollment and is presented to professors at the start of each semester/term. Students must
attend only those classes/sections as printed on the registration form.
2. Refund of fees or voluntary withdrawal shall be in accordance with existing school policy.
3. This is an official school document. Unauthorized alteration of this form is strictly prohibited.
4. A student who transfer or withdraws, within two (2) weeks after the beginning of classes, and who has already paid the pertinent tuition and
other school fees in full or for any length longer than one month, maybe charged 25% of the total amount due for the school term if he
withdraws within the 1st Week of classes, or 50% if within the second week of classes, regardless of whether or not he has actually attended
classes. The student may be charged for all the school fees in full if he withdraws anytime after the 2nd week of classes. (Section 100.
