DCEP: Dexamethasone, Cylcophosphamide, Etoposide, Cisplatin 1/2

Diagnosis: R/R Multiple Myeloma


Allergies:
Height: _______cm Weight: _______kg BSA: _______m2

Day 3= ___/___/___ Day 4= ___/___/___

Pre-hydration: 1L 0.9%NaCl at 250mL/h prior to chemo on day 1

Hydration: 0.9% NaCl at 1L/m2/day= _______mL/h to infuse


during chemo starting on day 1 and until 12h post completion of
chemo. Maintain urine output 100mL/hr during chemo. Advise
patient of adequate PO hydration once chemo & IVF complete.

PRN Anti-emetics:
Prochlorperazine 10mg PO q6h prn mild nausea/vomiting
Ondansetron 4mg IVP q4h prn moderate nausea/vomiting

Emesis prevention:
Give Ondansetron 8mg IV Push q12h on day 1 to day 4 for 8
doses. Start 30 minutes prior to first dose of chemotherapy.

Chemotherapy regimen:
Give Dexamethasone 40mg +50mL 0.9%NaCl IVPB once daily
over 30 minutes on day 1 to day 4 for 4 doses. Give 30-60 minutes
before starting each dose of chemotherapy.
DCEP 2/2

Chemotherapy regimen continued:

Give Cyclophosphamide 400mg/m2/day= _______mg


Continuous IV infusion over 24 hours on day 1 to day 4 for 4
doses. (total 1600mg/m2/cycle)

Give Etoposide 40mg/m2/day= _______mg Continuous IV


infusion over 24 hours on day 1 to day 4 for 4 doses. (total 160mg/
m2/cycle)

Give Cisplatin 10mg/m2/day= _______mg Continuous IV


infusion over 24 hours on day 1 to day 4 for 4 doses. (total 40mg/
m2/cycle)

Mix cyclophosphamide, etoposide, and cisplatin in 1L 0.9%NaCl.


Infuse each dose over 24h via 0.2micron in-line filter.

Supportive Care:
Filgrastim (in-patient) or Pegfilgrastim (out-patient) to be ordered
by HCP on EMAR as indicated >24 hours after completion of
chemotherapy
