Discharge Letter for Mr. Gerald Baker

Re: Mr Gerald Baker, aged 79 years old

I am writing in behalf of City Hospital to discharge back Mr Baker in your fulltime care
tomorrow. She was admitted on 3 September 2010 due to hypertension and underwent Left
Total Hip Replacement.

Upon assessment after the surgery, he exhibited signs of disorientation attributable to


anesthetic effects but became coherent afterwards. He also reveals signs of anemia but became
stable after receiving blood transfusion. Mr Baker was prescribed to take Aspirin
postoperatively, Ramipril, which is indicated for hypertension and Panadeine Forte, which is
recommended for pain if necessary.

His nursing management in our facility initially consisted of daily dressing on the incision site
and performing strengthening exercises, which resulted to his good mobility postoperatively.
However, following his discharge, it is important to preserve the skin integrity, to monitor
Panadeine Forte intake and to assist him in exercise program. Equipment required for the
program will be provided by the hospital.

Your further assistance in Mr Baker’s recovery would be greatly appreciated. Should you have
any clarifications, please do not hesitate to contact me. Thank you for your careful attention to
this letter.
