Ma’am,

This is with regards to our request to your good office for the approval of two (2)
Volunteers of this unit, to be hired as Prison Guard in Job Order Status of
employment.

Among the five (5) who have rendered their services as volunteer Prison Guards
this two (2) are the ones who passed the assessment test given to them.
Attached herewith are their volunteer service agreement and assessment results.

For your ready reference, the names of the same is hereunder listed together
with the names of Job Order Personnel who resigned to be replaced by them, to
wit;
