individuals, who’s physical functioning has been affected. Our mission at Complete Care is to provide relevant,
reliable, and transformative resources to maximize patients’ quality of life and level of independence, through a
holistic approach.

Program Philosophy: At Complete Care we believe in providing an inclusive, diverse, and judgement free
environment to foster individuals’ rehabilitative journeys. We believe through our patient-centered approach
individuals will be uplifted by a continuum of support to enhance their overall functioning, quality of life, and
independence. Creating confidence, security, and lifelong skills to every individual who steps into our facility.

G1: Provide exceptional quality care for our patients.
O1: Research, analyze and apply at least 1 evidence-based practice each quarter, where
applicable.
G2: Build cooperative relationships with patients, their families, and other members of the treatment
team.
O2: After initial assessments have been completed, the treatment team will meet with patient
and/or family members within 72 hours of admission.

Potential for Independent Living: Through our resources, we hope to provide an individual with the ability to
have the best quality of life and a high level of independence.

Transition Services (offered/available):


We offer transition services for both outpatient to inpatient and inpatient to outpatient. If individuals are being
treated as outpatients at our facility, they can transition to inpatient if they need our LTC services. With our
team of providers, they will assist a plan for clients. When patients receiving inpatient services are preparing for
discharge, we will prepare community program(s) and service(s) referrals to support their continued care, as
well as resources to continue rehabilitation through outpatient services which will be facilitated by our team of
providers.

Our facility can serve up to 100 patients per year.

Average Length of Stay and/or Average Length of Treatment:


Until an individual no longer needs care. Length of treatment (inpatient/outpatient) can vary based on the
severity of diagnosis; however, our inpatient and outpatient services accommodate for longer duration
treatments.

Demographic of the community.


a. Culture:
Complete Care is located in the coastal port city of Wilmington, North Carolina. Wilmington has
warm weather and extraordinary beach views. The community is 71.7% Caucasian, 18.1% African
American, 4.67% White Hispanic, 2.02% Multiracial, 1.41% Asian, .28% Alaska Native and
American Indian, and .82% other Hispanic. Wilmington is a primarily English-speaking city, but
there are some residents who primarily speak Spanish as well.  The community is 77% Christian,
20% unaffiliated, and 3% non-Christian faiths. The major working industry is healthcare and social
assistance with retail trade as the next largest industry. Making the median annual income in 2018
$52,756. 
The locals enjoy being outdoors, taking care of the environment, and going to one of the main
three beaches that are located close to every resident. The three main beaches include: Wrightsville
Beach, Carolina Beach, and Kure Beach. The typical clothing worn in Wilmington can be described
as casual Southern. This includes shorts and tee shirts being a common sight. Wilmington consumes
a large amount of local seafood as it is located on the coast within minutes of local beaches.
Wilmington residents also make it a point to support their local, and small, businesses such as
Island’s Fresh Mexican Grill, Grinder’s Cafe, and more. Wilmington is home to the University of
North Carolina at Wilmington so residents show their Wilmington pride by attending collegiate level
athletic events when home games are offered. Wilmington is also home to Riverfest which is a
festival that appreciates the Cape Fear River in Downtown Wilmington, or on the
Riverwalk. Another local festival is the Azalea Festival which was started by Dr. Houston Moore in
1934. Dr. Houston wanted to beautify and landscape Greenfield Lake so it could be turned into a
park. He believed there should be a festival commemorating when the park begins blooming each
year and thus the Azalea Festival was born. The first ever one was held in 1948. Resulting in
Wilmington being a city that offers numerous leisure activities, a sense of community, and strives to
uphold the beauty and history of its origins.
