Nursing Interventions for High BP

The patient has high blood pressure and is non-adherent to their treatment regimen due to a lack of knowledge about their condition and treatment. The goal is for the patient to adhere to their treatment plan. Nursing diagnoses include deficient knowledge and intentional/unintentional non-adherence. Nursing interventions include educating the patient on their condition, treatment plan, and importance of adherence. The expected outcomes are that the patient understands their condition and treatment plan and participates in activities to adhere to their regimen, such as following a restricted diet and reducing cigarette consumption. Evaluation will assess if the patient meets or partially meets the goals.
