COMPLETE CHECKLIST BELOW TO ENSURE A SAFE LIFT IS PLANNED YES NO
1- Raise all necessary permits and authorizations.
2- The load weight is confirmed known.
3- Conduct tool box talk with all relevant persons and conduct site assessment.
4- Verify all equipment is in acceptable condition and all certification is in order.
5- Cordon / barricade off designated area.
6- Ensure all safety devices are operational.
7- Outrigger pads are fully extended and blocking is sufficient for the load.
8- The load hook is directly over the load center of gravity.
9- Boom angle, boom length, lift radius, and the crane capacity are known.
10- Tires are clear of the ground and the crane is level.
11- Ground, soil, and/or pavement is confirmed to have capacity for the imposed load.
12- Rigging equipment has been inspected and in safe working condition.
13- Crane operator, remain alert and NEVER leave the controls at any time during the lift.
14- Under the direction of the appointed banksman, slow and safely lift and transfer the cargo to its final
15- Once the lift is complete and when safe to do so, remove rigging & lifting tackle for cargo.
16- Return / properly stow equipment, tidy and clean the site and remove barricades.
17- Confirm lifting rigging / tackle is fit for use & fit for purpose. Inspected by competent person prior to lift.
18- Ensure personnel working at height adhere to Galfar working at height / fall protection requirements.
19- Confirm with all personal readiness to proceed.
20- Ensure all parties are fully aware of the procedure to follow in case of emergency shut down / loss of power.
21- Lifts in close proximity to power transmission lines shall meet requirements.
