The aim of the procedure is to give guidance on how an issue based risk assessment must be
conducted.

6. INTRODUCTION
The process needs to be simple enough to satisfy the objective of having the Line Manager/
Supervisor conduct an Issue Based Risk Assessment (with the facilitation from the “Specialists”
where and if needed) but at the same time it needs to be comprehensive enough so as to ensure
an acceptable degree of accuracy.
7. NEED
The Mine Health and Safety act requires that Risk Assessments must be conducted. In conducting
Risk Assessments Hazards will be identified and controls can be put in place to prevent workers
from losing hours.
8. MANAGEMENT OF RISK
Measures for the management of risk should reflect the principle of eliminating the hazard in a pro-
active manner where practicable, followed by risk reduction exercises (either reducing the
likelihood or occurrence of the potential risk). Adopting personal protective equipment (PPE) will be
seen as the last line of defence. Hazard Identification, Risk Assessment, and Determining of
Controls are key elements to the management of risk.
As and when required specialists will be used to conduct more in depth / special risk assessments
such as FMECA, HAZOP, and FMEA.
9. HAZARD IDENTIFICATION
South Africa Region Services shall establish, implement and maintain a procedure for the ongoing
pro-active hazard identification, risk assessment, and determination of necessary controls to deal
with the hazards and associated risks. The process for hazard identification shall take the following
into account:
• Document and keep the results of identification of hazards, risk assessments and
determined controls up-to-date.
• Ensure that the OH&S risks and determined controls are taken into account when
establishing, implementing and maintaining the South Africa Region Services Safety
System.
• Ensure that results from Issue base and continuous risk assessments are taken into
consideration before controls are altered or implemented.
• Included in objectives and targets.
• Included in standards and operational procedures.
• This information is fed back at meetings by the appropriate levels of management for the
evaluation of its OH&S objectives and adjustments - particular regard should be given to
information or data from those most likely to be affected by individual OH&S objectives, as
this can assist in ensuring that they are reasonable and more widely accepted. It is also
useful to consider information or data from sources external to the organization, e.g. from
contractors or other interested parties.

10. PROCESS OF REVIEWING / UPDATING / CONDUCTING RISK ASSESSMENTS
• The Baseline process has identified and focused on Significant Risks (Agency, Occupation,
Activity/Task Health/Hygiene Factor)
• New Equipment is to be introduced e.g. A new type of pump is installed
• After a serious Accident, Incident or Occupational Illness has occurred which affects a specific
IBRA
• A new facility is being developed
• A task needs to be reviewed
• A change in process is introduced e.g. the use of Electric Drills vs. Pneumatic Drills.
• A request is received and is sanctioned by the Health and Safety Committee
• Any amendments or new legislation as and when required
• A management of change requests an Issue Based Risk Assessment.
Once the need to do an Issue Based Risk Assessment on a certain issue is identified the
Manager/HOD must define/mandate the assessment if needed.

11.1 Mandate / Define the Assessment
It is recommended that to select a team from the workplace being reviewed by taking a
vertical slice through the management structure to the lowest levels. This will include the
frontline supervisor and the team from the workplace being assessed.
Where necessary, specialist expertise should also be brought in.
The facilitator will oversee the process of identifying hazards by making use of several memory
jogging documents such as :
• A list of applicable occupations
• A list of agencies (materials, equipment, tools, etc.)
• A list of Hazardous Chemicals
• A list of Hazards, etc.
• Incident stats “History”
• Legislation specific to task
• Previous Risk Assessments

11.4.1 Process – Worksheet
Ensure that the Risk Assessment Evaluation Checklist (Annexure 6) is used throughout the
process as this will assist in the identification of Hazards, Causes, Controls in Place etc.

Ask the question, “What can go wrong when performing this task step using the equipment and
material identified for the task step in the work environment as identified.” List these as the
hazards for the task step.
• Causes Column
List the causes for the hazards as identified.
Note: NEGLIGENCE is not a cause. What the person does or did not do to be negligent, is the
cause.
• Consequence Column
List the consequences to People, Equipment, Material, Environment and Process should the
hazard result in an incident.
• Controls in Place Column
List the current controls in place for the task step, equipment, material and work environment.
• With Controls in place Column
Using the AngloGold Ashanti 48 Block Matrix to Determine Risk Index, determine the risk
ranking for the task step.
• Recommendations Column
List the recommendation to address / eliminate the CAUSES as listed in the Causes Column
for the hazard (If we can eliminate the CAUSES, we will eliminate the HAZARD).
• After Implementation Column
Using the AngloGold Ashanti 48 Block Matrix to Determine Risk Index, take into consideration
the effects the recommendations made will have on the CAUSES; determine again the risk
ranking for the task step (It would be ideal if this could be scheduled for a later date to test the
effectiveness of the implemented recommendations).

11.4.2 Process – Action Sheet
Repeat the process as described above for all the task steps as set out in the Process Flow.
Once all the task steps have been assessed, complete the Assessment Summary.
12. IMPLEMENTATION
• It is the responsibility of the Manager / HOD to forward a copy of the “Action Manager” (Action
Plan) to all the identified responsible persons.
• The original must be handed to the Systems/Document coordinator who will enter the actions
required into RMS (Risk Management System) or a manual follow up system used for sign off
and filed as per the Document and Data Control Survey.
• The final signed off IBRA to be loaded into SharePoint
• Baseline must be updated by the responsible department wrt Risk rankings & IBRA number
• Identified responsible persons who must update the different “Risk Control” elements.
• Responsible Manager / HOD to follow-up and ensure implementation and compliance

Similarly, where hazards were identified, it only reflects the opinions and experience of
the team members and does not imply that no other, further hazards exist. The Risk
Assessment Facilitator attempted to provide the best possible guidance to the team
members in identifying possible hazards and formulating feasible recommendations,
but at no stage attempted to be prescriptive.

DEFINITIONS:
DOCUMENT
Information and its supporting medium (The medium can be paper, magnetic, electronic or optical
computer disc, photograph or master sample, or a combination thereof.)
RECORD
Document stating results achieved or providing evidence of activities performed
CONTROLLED DOCUMENT
Any document that needs to be controlled in terms of its distribution and use, such as the Policy
and documented procedures. These documents are either signed in original ink and/or signed for
on a distribution list. These documents will be signed off after evaluation has been done to
determine that they are adequate for purpose. The document will be signed off by the relevant
members as depicted on the distribution list.
Once copies are printed of the database, they are considered to be “uncontrolled”.
OBSOLETE DOCUMENTS
Documents that have been replaced by later revisions or those that is no longer relevant or valid,
and has been cancelled from the system. These documents will be identified either by crossing
them out and writing obsolete or rubber stamp obsolete document.
UNCONTROLLED DOCUMENTS
Any documents that have not been issued under the circumstances under “controlled documents”
are categorised as uncontrolled. This includes, but may not be limited to:
• Documents that do not need to be controlled such as the monthly safety topic.
• Documents not distributed by the authorised person as per the procedures “authority and
responsibility” table and signed for by the recipient.
• Documents printed from the electronic database. (Watermarked “uncontrolled document”)
• Documents duplicated or photocopied from controlled documents.
• Photocopies of the Occupational Health and Safety policy as issued to the public or other

It is not possible to judge from an uncontrolled copy whether it is the latest version. It is the

SEVERITY / CONSEQUENCE
Outcome of an event. There may be one or more consequences from an event. May be
expressed qualitatively or quantitatively may range from positive to negative (Speculative).
FREQUENCY
Occurrence per unit time, (May be expressed qualitatively or quantitatively).
EXPOSURE
How often and for how long employees are exposed to a hazard/s.
LIKELIHOOD
Means the chance of an event occurring.
DUE DILIGENCE
Taking reasonable care to protect the health and safety of all employees. Provide equipment,
maintain the equipment, use equipment as prescribed, provide information relating to the
equipment, and provide competent supervision.
INCIDENT
An undesired event which under slightly different circumstances could result in harm to people.
Damage to property or loss to process or an undesired event that could or does result in a loss.
RISK MATRIX
A Risk index can be determined by plotting likelihood and severity indices on the y and x-axis
respectively and then using them to obtain a risk ranking.
HIRA
Process of recognizing that a hazard exists and defining its characteristics
• H = HAZARD
Anything around us that we can see as well as those energy sources we cannot see e.g.
Gas and radiation that can cause harm
• I = IDENTIFICATION
Identify the significant hazards (Process and recognition)

PROCEDURE
