Continuous Kardex update, Head Nurse’s rounds

8:30 AM – 9:00 AM Pseudo-charting and charting


9:00 AM- 9:30 AM Carrying out due medications
9:30 AM-12:00 PM Pseudo-charting and charting
12:00 PM – 12:30 PM Carrying out due medications
12:30 PM – 1:00 PM LUNCH BREAK
Vital signs taking and plotting
1:00 PM –1:20 PM
Preparing due medications

Rechecking patient’s chart for proper charting and carrying out of due medications.
1:20 PM – 1:35 PM
Kardex update after Dr.’s round

Giving due medications. Taking I and O, plotting and charting.


1:35 PM – 1:45 PM
Rechecking the patient’s chart

1:45 PM – 2:00 PM Endorsement for the next shift
