Leave Application Guidelines and Form

This leave application form collects essential employee information like name, designation, department, contact details and client/project details. Note: Approval must be taken 7 days prior in case of short term leaves (Max 3 Days) and 1 Month prior in case of long term leaves (3
Days +).
Please submit the form after getting approval & sign of HOD / concern person.

Guidelines
o “LEAVE APPLIED SHOULD NOT BE CONSTRUED AS IF LEAVE IS GRANTED”
o All leaves are granted at the convenience of the management. Nothing can limit the free discretion of the management to
refuse, revoke or curtail leave, subject to the exigencies of work.
o The Employee must resume his duty on the expiry of his sanctioned leave.
o The employee concerned should get his leave checked/Verified regarding leave due by the H.R. Department so that this
will be easier for the Department Head for sanctioning of the leave.
o While on this it should also be noted that if there is weekly off day or paid holiday falling in between the days of the sanctioned leave
that weekly off / and or holiday will also be considered as total leave days granted.
o If the weekly off or paid holiday is before or after the sanctioned leave the workmen / Employee must present himself either the day
before or after that holiday or else that day will be treated as absent for those days.
o It is the responsibility of Manager and Supervisors to ensure correct leave documentation is submitted to payroll and it is the
responsibility of staff to comply with policies with respect to leave documentation.
o In case the Employee remains absent without getting his leave sanctioned in advance or remain absent beyond the period of leave
originally granted or subsequently extended, he shall lose his lien on his job and shall be deemed to have voluntarily
abandoned his services on his own accord, unless he returns within 8 days and explains to the satisfaction of the management the
reason of his absence or inability to return on the expiry of the leave as the case may be.
o Please be insure that office are not left entirely an attended during office hours, regular punch the Entry Card on main door access, any
employee if found more than 15 minutes in a day unattended during office hours will viewed seriously and penalties shall apply.
o It is reiterated again to maintain the discipline of the shift working by being self-disciplined.
