AFFIDAVIT

I, YUMNA N. KHAN, D/o. NASIR KHWAJA, R/o. 54/1, 9th COMMERCIAL


STREET, PHASE 4, D.H.A, KARACHI, do hereby state in oath as under:-

1. That no Criminal Proceeding or Proceedings for professional misconduct has ever


been instituted against me in Pakistan or outside of Pakistan in any court of law.

2. That I have passed the following exams as under:-

a. O’ Levels Examination in the year 2011


b. A’ Levels Examination in the year 2013
c. L.L.B Examination in the year 2016

3. That the contains mentioned in form ‘A’ are true and correct & I was not engaged
in any Business, profession, services or vocations.

4. That during the period of my pupilage I was not engaged in any business,
profession, Vocation or service.

5. That I say that incase it is found after the Enrolment that I have made a mis-
statement regarding my engagement (s) some other employment, full or part time
job, business, Government, Semi Government or Private Sector or Vocation, my
license to practice as an advocate be cancelled forthwith, without any notice,
besides criminal proceeding against me in accordance with law.

6. What ever is stated above is true and correct to the best of my knowledge, faith
and belief.
