ACKNOWLEDGEMENT

First of all we would like to thank our Almighty God for the gift of knowledge, wisdom,
and good health in conducting this case study. We also thank our Lord Jesus for the opportunity
to meet this study and for us to enable to learn about the different illnesses inside the Surgical
and Medical Ward through previous and current experiences, and for us to enable to apply the
knowledge we have acquired from school and clinical exposure in the Ward.

Second, we thank our client, Mr. SD and his mother, for their cooperation, and for
entrusting us their privacy. We owe them a lot in this case study that we’ve done.

Third, we would like to express our sincere gratitude to all our Level III clinical
instructors most especially to our Point Clinical Instructor Ms. Mariz Rochelle C. Sedon, RN,
MAN, for guiding us and molding us. Also, for all the patience as well as the wisdom and skills
they all have imparted to us that are very helpful in achieving the purpose of this case study.

Fourth, we would like to thank the BPMC staff, particularly the nurses at the Surgical
and Medical Ward nurse station, for being friendly and helpful to us.

Lastly, we would like to thank our beloved families for their outmost support. They have
not only provided us financial assistance, but also emotional support and their prayers.

May our Lord God Almighty continue to shower us His blessings and guide us through it
all! Praise be to God!
