1. Go to [Link] and login name verifier account
2. Click “Name Verifier” menu
3. Click print icon to view the details of the application. Verify the proposed name in CRS
and other listings
3. After verifying the name, click process icon.
3. Accomplish the form appropriately and click Save.
Notes:
• If name is approved,
encode/reserve the name
in CRS via the amendment
module.
• Check the indicated
processing office in the
application form. Make
sure to assign the
application form to a
processor from the
indicated SEC Office.
• Review the form before
clicking save.

2. Click “Application Processor” menu
3. Click print icon to view the details of the application. Check for the completeness and
correctness of entries.
3. After reviewing the application, click process icon.
3. Accomplish the form appropriately and click Save.
Notes:
• If application is approved, create a
PAF and save it as PDF file. Then
upload the PDF file on the field
provided.
• If the name is disapproved, the
status of application is
automatically set to disapprove.
• If status is disapproved, make sure
to indicate all deficiencies found in
the application form.
• Review the form before clicking
save.
