INSTRUCTIONS: This questionnaire consists two parts. Read each question very carefully. Answer legibly, clearly, and concisely. Observe a
one-and-a-half-inch left margin and an inch right margin. You may write your answers continuously on the same page of your answer sheet.
Leave three writing space rows in between answers to each item and a writing space row in between paragraphs in your answers. A mere
