I, Abnel G. Dabe, of legal age, married, Filipino, and a resident


of Brgy. Samput, Paniqui, Tarlac, after being sworn to in accordance
with law, depose and state that:

1. I am the Petitioner in the instant case;

2. I have caused the preparation and have read and understood all
the allegations contained in this Petition and found that the
allegations therein are true and correct according to my own
personal knowledge and information, belief and there is good
ground to support it, and based on authentic records, whenever
applicable;

3. I have not commenced any action or filed any claim involving


the same issues in the Supreme Court, the Court of Appeals, or
any different division thereof, or any other tribunal or quasi-
judicial agency, and to the best of my knowledge, no such
action or proceeding is pending in the Supreme Court, the Court
of Appeals, or of different divisions thereof or any other
tribunal or agency; and

4. I undertake to inform the Board of any action and its status, if


there be any or should I learn that a similar action or proceeding
have been filed or is pending within five (5) days from notice.
