Panvel Dist Raigad From the date 01/01/2020 onwards.

I have submitted Voter ID/ Pan Card / Driving Licence/ Adhar Card /Passport in support of my/ identity
proof.

I have submitted agreement of purchase / lease / rent of the place of business / telephone bill /
electricity bill as the document in support of place of business

(Board resolution in case of any Board / Trust/ Corporation/ Autonomous Body should be attached
where necessary)

I submit that I have or will engage maximum number of 01 employees during the business and shall pay
fees accordingly as prescribed by the Govt. I submit and declare that I will not undertake any illegal
activity or any business prohibited in law in force in India.

I declare that the place of business is not located in any area wherein commencing / running of such
business is prohibited by any law or order of any Competent Authority

I submit that the registered address of the establishment is as follows;

I / We are aware that the registration of the establishment under Maharashtra Shops and Establishment
Act, 1948 does not proclaim or restore any right of ownership or possession or any right to property. The
said document shall not be used for the purpose of enforcing any right relating to the property.

I undertake to abide by the provisions of the Maharashtra Shops and Establishment Act, 1948 and the
Rules and orders passed there under by any Authority.

I hereby declare that the particulars and the statement made in this application are true and correct to
the best of my / our knowledge and belief and nothing has been concealed or held therefrom.

I / We fully aware that any information furnished in the application if found in- correct or false or
misleading or obtained by misrepresentation will render the cancellation of the Registration Certificate
and shall render me / us liable for any penal action or other consequences as may be prescribed in law
