 list and number of job titles/positions being requested by the company indicating
preferred salaries and benefits
 list only positions and quantity allowed by Kuwait government in visa quota or visa
 contract of duration
 regular working hours

 food
 accommodation and transportation
 medical insurance
 air tickets (round trip)
 probationary period

 Special Power of Attorney

 Employer’s company and address indicated
 Name of Madam is indicated
 POEA License is correct/updated
 Signed by employer and madam
 Notarized

 Recruitment Agreement
 all pages must be signed with stamp by employer and madam

of Commerce, Ministry of Health, or Ministry of Interior)

 Copy of Civil ID of employer/authorized signatory with original English translation
 Copy of visa quota with original English translation
 copy of valid PRA License
 Copy of Passport of Owner of PRA
 Authorization – Letter authorizing a person to transact with POLO regarding the job order
and copy of the authorized person’s civil ID. – template attached
 List of Filipino Workers currently working in the company with phone numbers
