AFFIDAVIT

I, PAUL NOEL GARACHICO CAJIPE, Filipino citizen, of legal age,


married and presently residing at 106 Marang Street, Project 2, Quezon City,
Philippines, after having been duly sworn in accordance with law, hereby depose
and state: THAT

1. I am the uncle of KYLE JEREMIAH MINDO CAJIPE (Kyle for


brevity), also of legal age, Filipino citizen, single and presently residing No. 5
McAdams Road, Barangay Mayamot, Antipolo City, Philippines

2. Kyle is a member of a Filipino dance group “Autonomicass”, who is


scheduled to compete in an international dance competition World Supreme
Battleground (“WSB”) to be held in Sidney, Australia on September 9 – 11, 2016

3. I hereby declare that I shouldered Kyle’s transportation expenses in going


to and from Sidney, Australia and that I also declare that I will support Kyle’s
other expenses in his trip to Sidney, Australia.

4. That I am executing this affidavit to attest to the truth of the foregoing as


well as for whatever legal purpose it may serve.
