ACTIVITY/ PROGRAM/ PROJECT PROPOSAL

a. Title : (Indicate the title. Ensure that it is original or


attuned to the program title of cooperating agencies)
b. Implementing Organization/s : (indicate the name of your org and other
cooperating agencies, student organizations, and or individuals)
c. Location : (indicate the target location/s of the project)
d. Duration : (indicate the length of the activity e.g. no. of
days or hours; and the intervals e.g. every Saturday, twice a month, etc.)
e. Starting Date : (indicate the target date when the activity will
start)
f. End Date : (Indicate the target date when the activity will
end)
g. Budgetary Requirements : (indicate the amount necessary for the activity
to proceed)
h. Source of Budget : (indicate where the money will come from e.g.
Membership funds, CAS Fiduciary Funds, Sponsorships, etc. )
i. No. of Beneficiaries : (indicate who are the beneficiaries and the
estimated minimum target of people who will benefit from the activity)

I. Background and Rationale


This section should provide the following:
 A problem or critical issue which the proposal seeks to resolve
 How the project is relevant to the VMGO of CNSC and responsive to
local, regional, national and/ or international development strategies,
programs or policies
 Whether there are other programs and activities the proposal will
complement
 How the need for the project was determined. (Is there an existing
study? Was there a request submitted to your organization? etc.)
 Description of the target beneficiaries
 What kind of assistance will your organization provide through this
proposal. Be specific.
 What makes your organization fit to provide such services. What
relevant experience and capabilities does your organization have to
share resources or expertise?

II. Objectives

This section is divided into two parts: development objectives and immediate
objectives.

A. Development Objectives
This section should list the objectives that form part of national
development strategies and policies. (i.e. Buwan ng Wika
objectives from the Komisyon sa Wikang Filipino,
environmental projects can come from the objectives of DENR,
etc.)
The discussion should indicate specific social, cultural,
economic and/or academic objectives to which the proposal, if
successful, is expected to contribute to the target beneficiaries
and to the larger community.

B. Immediate Objectives
This section should describe what the project is supposed to
achieve in terms of its target beneficiaries. What changes do
you intend to make?

III. Expected Results


This section should discuss the overall results that the project is expected to
accomplish, enumerate possible unintended effects (whether good or bad) or
challenges; and possible mechanisms to address obstacles.

The discussion should indicate measurable outcomes and as such, must


contain quantitative terms as much as possible.

IV. Project implementation and management


This section is divided into three parts: Project Beneficiaries, Project activities
and work plan; and Project Monitoring and Evaluation

A. Project Beneficiaries
Describe who and how many people are expected to benefit from the
project, both directly and indirectly. Explain their involvement in designing
the project and the expected role in project implementation and
evaluation.

B. Project Activities and Work Plan

This section should be in tabular form.


If the proposal is a huge project, list all subsequent activities included in
the project, their timing and duration, and people responsible (heads) for
each activity.

Include here the list of committee heads and members, work plan (could
be a Gantt chart), calendar, etc)

C. Project Monitoring and Evaluation
This should discussed proposed mechanisms and procedures to monitor
the operations of the project to ensure that activities go as planned and
towards the achievement of the objectives, and that appropriate corrective
measures can be instituted when required.
This should discuss who will be responsible for periodic progress reports
and the accounting of expenditures. This should also indicate how target
beneficiaries will be asked for feedback (i.e. evaluation sheet, focus group
discussion, interview, etc.)

V. Budget
Discuss here the budget required for the project. Use the following table:
