TERMINATION NOTICE

Last ___________________, we sent you a notice about your preventive suspension


regarding the following committed acts which are your duties and responsibilities which
are considered inimical to the interest of the Company and its customers/clients,
particularly in the following instances:

You were given a chance to explain in writing within _____________- days upon
receipt thereof why no appropriate disciplinary action in connection with the foregoing
acts and misconduct shall be sanctioned against you but failed to do so.

In addition, even without the explanation letter, further investigation revealed


that the foregoing acts were found to be committed by you. Thus, we regret to inform
you that effective today, you are hereby fully terminated of your duties and functions in
the company.
