1. Quotations are invited Placement of Subject equipment on Maintenance Contract


with Spare Parts for the Period of 3 [Link] per terms and conditions mentioned
below, which may be read thoroughly before submitting your offer.
2. Submission of Tender - The quotations must reach this office on 15 Jan 2018
and time upto 1100 hours:
3. Page number must be endorsed on every page of quotation.
4. Sales tax is exempted on supply of goods to hospitals vide Govt of Pakistan
Finance Bill 2008. Therefore, do not include sales tax while quoting your rates.
5. 100% payment will be divided equally in 6 parts and made on half yearly basisfor
the period of 3 years.
6. Firm should also give References of Customers with whom such types of
contracts are made.
7. Firm should also give list of engineer along with their certifications that they are
trained to carry out repair work of Subject eqpt.
8. Please note that this is only an invitation to tender and no commitment to you.
The Hospital Management also reserves the right to accept or reject all or any
offer without assigning any reason for such action.
9. Firm’s poor response / performance regarding supply of equipment and repair
backup support for any equipment already installed in any FF Project may render
the firm ineligible for short listing.

Terms & Conditions of Contract:

1. SCOPE OF WORK
1.1 This contract includes all the repair work and replacement of defective spares including
all vacuum elements. Except the parts mentioned at Anx-I.
1.2 The periodic maintenance/service shall include the following:
a) Function, operation and safety checks.
b) Adjustment, calibrations and lubrications.
c) Correction of detected defects which result from normal wear and tear which may
involve the replacement of spare parts. Performing preventive work where this is
seemed necessary by the firm.

1.3 Keeping in view the nature of the equipment, maintenance/repair work shall only be
carried out by Firms authorized personnel. Access of un-authorized persons to the
equipment shall release the Firm, of the maintenance/repair obligations and any
subsequent risk arising there from.

1.4 The equipment shall be made available for repairs whenever the need arises.

1.5 Planned preventive maintenance (PPM)/ modifications shall be performed during the
week days (Monday till Saturday) as per the principal’s recommended schedule.

2. SPARE PARTS.

2.2 Firm, at all times during the term of this contract shall ensure the availability of the parts
which are required to keep the equipment in working condition.

2.3 Each time failure occurred Firm’s engineer will asses the system and part shall be
arranged by Firm.

2.4 It is agreed that if any part or parts of the equipment become unserviceable, Firm will
arrange for its replacement on top priority basis.

2.5 Firm shall replace the new part/s with the defective part/s.

2.6 The parts will reach Karachi/Pakistan within 96 hours after the same has been ordered.

2.7 UPS/Power DB is not included in the maintenance contract.

3. SYSTEM UPTIME.

3.1 Engineer shall respond to service call within 3 hours during the normal working hours
from 09:00 - 18:00.

3.2 Firm Guarantees 90% accumulative uptime for the contract Period annually.

3.3 Uptime percentage will be calculated by dividing “system in service Hours” by


“Available Hours both measured on 08 hours/day basis. If the equipment fail to achieve
90% uptime, then the extend down time hours will be added to the contract period at NO
extra cost to Firm on the following conditions;

Between 80 to 90 % 1.5 times the additional down times hours


Less than 80% 2 times the additional down times hours

3.4 Down time is defined as failure of equipment that prevents the system from acquiring,
processing and displaying the images data such that the procedures can not be performed.

4. MAINTENANCE CHARGES AND MODE OF PAYMENT

4.1 In consideration of the performance of Firm under this contract, SHAUKAT OMAR
MEMORIAL HOSPITAL, FAUJI FOUNDATION shall pay to Firm the total amount
equally divided in 6 installments as maintenance charges with spare parts for a period of
three years on completion of every 6 months.

5. MISCELLANEOUS

6.1 The maintenance, repair and parts replacement agreement includes field modifications
recommended by the factory. Any modifications or relocation of the equipment if
requested by the Client is the responsibility of the firm, if technically possible.

7. TERMINATION

7.1 Client has the right to terminate the contract with 30 days notice if Firm fails to provide
service as per contractual condition after being duly notified by Client in writing of its
short comings and given a reasonable time for remedial action.

5th installment: At the completion of 30 months from signing of contract.

6th installment: At the completion of 36 months from signing of contract.

The firm's engineer must also respond to service calls within 3 hours during normal working hours to support this uptime .

The contract allows Shaukat Omar Memorial Hospital to terminate the agreement with a 30-day notice if the firm fails to provide service as per the contractual conditions. This action must follow a written notification of shortcomings to the firm, allowing reasonable time for remedial action .
