Plaintiff, by and through the undersigned counsel, unto this


Honorable Court, most respectfully avers that:

1. In an Order dated November 11, 2013 and duly received by


plaintiff on November 19, 2013, hereto attached as Annex “A”,
this Honorable Court required plaintiff to submit the original or
certified true copy of the Secretary’s Certificate authorizing
Atty. Catherine C. Fontanoza to represent it in this case, with
documentary stamp tax duly affixed thereon as required under
Section 198 in relation to Section 201 of the National Internal
Revenue Code within five (5) days from notice thereof.

2. In compliance with the Order, plaintiff respectfully submits the


original copy of its Secretary’s Certificate with the
corresponding documentary stamp tax duly affixed, hereto
attached as Annex “B”.
