I, April R. Segiente, hereby grant my wife, Venus F. Segiente, permission to act as my


representative with regard to my birth certificate, including application for a copy and collection
of such copy.

Any and all acts carried by my wife on my behalf shall have the same effect as acts of my own.

I am also sending my two identity proofs (Voter’s ID and SSS’ ID) for identification purposes.

Any and all acts carried by my daughter-in-law on my behalf shall have the same effect as acts of
my own.

I am also sending my one identity proof (Senior Citizen’s ID) for identification purposes.
