AFFIDAVIT OF LEADERSHIP

I, BLSTAR CYDRIC M. JOSOL, Filipino of legal age, and a resident of Davao City after
been sworn to in accordance to law hereby disposes and say:

1. That am the President of CFC YOUTH FOR CHRIST of the University of


Southeastern Philippines, Iñigo St. B.o Obrero, Davao City.
2. That I am a 2nd year student under the program of Bachelor of Science in
Geology.
3. As part of my Duties and Responsibilities, I shall be liable for the official actions
of the said organization during my incumbency.
4. I am executing the affidavit to attest to the truthfulness of the foregoing, as a
Requirement for the (renewal of) recognition of organizations by the University of
Southeastern Philippines and whatever legal purposes it may serve.

In witness whereof, I hereunto affixed my signature this 30th day of August 2019 at
Davao City, Philippines.
