Notes:
Zayed Mungguraki was treated for acute meningoencephalitis as a result of a complication following mumps. The patient
was under your care at Children’s Emergency Department, Merton General Hospital.

Admitted: 13th April 2010
Discharged: 23rd April 2010
Country of birth: Tanzania
Diagnosis: Acute Meningoencephalitis

Social History
Parents: Hadia and Abdulasis, a migrant couple, arrived in Australia in 2008.
Employment:
Abdulasis: White Dove meat factory, shift worker;
Hadia: part-time house cleaner (for additional income)
Accommodation: Recently moved to rental accommodation
GP: No family doctor
Sibling: 3 year-old brother, Ali
Language: Kiswahili, Arabic Interpreter needs: Abdulasis has good spoken English but has limited writing skills;
learns English classes at an evening school. Hadia’s English is on survival level.
Financial advice: to increase income to support medical needs

Medical History: It is not clear which vaccines have been done on Zayed and Ali although both parents claimed there was
a vaccination record for the two children only that it has been lost. Parents do not remember whether or not Zayed and Ali
have received vaccines for mumps before.

Discharge Plan
Fully recovered from mumps and acute meningoencephalitis.
Will need advice on recommended vaccines for both children.
Will need neurological check-up—schedule a regular check-up with the Child Health Service GP

Writing Task:
Using the information in the case notes, write a letter to Charge Nurse, Community Child Health Service, 48 Johnson Street,
Jefferson requesting follow-up of the Munguraki family.

In your answer:
● Expand the relevant notes into complete sentences
● Do not use note form
● Use letter format

The body of the letter should be approximately 180–200 words.
