AFFIDAVIT OF DISCREPANCY IN NAME

I, JASON MANZANO DELOS SANTOS, of legal age, single, and with address at Zone 6 Rafael
Rueda, SR. POB (DISTRICT 1), under oath, state:

1. THAT my name is JASON MANZANO DELOS SANTOS as stated and registered in my birth
certificate issued by office of the Civil Registry of San Jose, Nueva Ecija.
A copy of my birth certificate is hereto attached as ANNEX “A”;
2. THAT I will be using De Los Santos stated in my Birthcertificate,
3. Instead of Delos Santos in my personal records, transactions and communications;
4. 3. THAT the name JASON MANZANO DELOS SANTOS and JASON MANZANO DE LOS SANTOS
refer to one and the same person, the herein Affiant;
5. 4. THAT I am executing this Affidavit to attest to the truth of the fore going facts and to use the same
for whatever legal purpose it may serve.
