JOINT AFFIDAVIT

We, MARILYN C. JAVIER, of legal age, Filipino, married and a


resident of Purok 4, Pinsao Proper, Baguio City and Barangay Tanod of
Pinsao Proper and CARMELA D. PILOR, Filipino, of legal age, married and
resident of Purok 2, Pinsao Proper. Baguio City and a Purok Leader of
Pinsao Proper after having been sworn in accordance with law, depose
and say:

1. That as officers of Pinsao Proper Barangay, we know all the


permanent residents of our barangay. One of whom is ROWENA CAMILET
BULATAO who is married to RENATO R. BULATAO;

2. That ROWENA C. BULATAO and her husband RENATO R. BULATAO


had been residing continuously at Pinsao Proper since 2002.

3. That ROWENA C. BULATAO died on August 10, 2019 at the Baguio


General Hospital and her wake is at the Resurrection Church at
Magsaysay Road, Baguio City which we attended for several days and is
yet to be buried this Sunday, August18, 2019.

4. That the certificate of ROWENA C. BULATAO indicates that her


CORRECT AND TRUE name was ROWENA MACASIEB CAMILET while in her
certificate of marriage her first name was wrongly entered as ROMWENA.

5. That ROWENA C. BULATAO and ROMWENA C. BULATAO are one


and the same person.

6. That we are executing this joint affidavit to attest to the truth and
veracity of the foregoing statements.

IN WITNESS WHEREOF, we have hereunto set my hands this 14th day


of August 2019 at Baguio City, Philippines.
