Philippine National Police (PNP) presently assigned at Lala Municipal Police Station with
office address at Lanipao, Lanao del Norte, after having been duly sworn to an oath in
accordance with law do hereby depose and state;

1. That I am the duly designated SEIZING OFFICER in the afore-mentioned


anti- illegal drugs operation;

2. That I made the markings of the property/ items/ articles seized during the
said anti- illegal drugs operation;

3. That the allegations contained therein are true and correct of my own
