2017 GCE N(T)-, N(A)-, O- & A-LEVEL EXAMINATIONS

Private Registration – Subject/Paper Entries Amendment


(Applicable for 2017 Private Candidates only)

1. This form is only applicable to private candidates who wish to make amendments to their subjects/papers
after they have registered for the examination and paid the examination fees.
2. This form may take you 5 minutes to complete and will require the following information
 Candidate’s Personal Particulars
 Subjects/Papers to be amended.
3. All fields are mandatory and must be completed in English in clear BLOCK LETTERS.
4. An amendment fee of $20 is chargeable in addition to the new subject fee. 50% of the replaced subject fee will be
refunded.
5. Please email this completed form to contact@[Link] by 24 Apr 2017 for GCE N(T)- & N(A)-Level examinations
and 25 Jun 2017 for GCE O- & A-Level examinations.
