FINAL PROJECT GUIDELINES
What: (1) Psychological Report (2) Treatment Plan
Where: PSYCH LAB, SBU (Department of Psychology, 4th Floor, St. Anselm’s Building)
When: on or before April 27, 2019, Saturday (7:30-10:30AM)
INSTRUCTIONS:
1. Please look for someone who is willing to undergo a psychological evaluation. This person must NOT:
be a family member, relative, close friend, or acquaintance
be a Psychology major of any college or university
be coerced or forced to participate
be disrespected or given discomfort throughout the process
participate unless the consent form is signed
It is encouraged that you ask someone who is not younger than 18 years old. If you wish to assess
someone below the age of18, consent must be provided by the parents/guardian. The nature and
repercussions of the project must be explained to them.
2. Take care of your participant. Review the standards and procedures of conducting a psychological
assessment. Make sure to conduct and administer everything professionally. You are reminded to
respect the rights and dignity of these individuals.
3. Safeguard all the documents. All test materials must be used and kept properly. Protocols, answer
sheets, questionnaires and recorded files of any format must be stored properly and securely. Protect
the identity and anonymity of your client.
4. Psychological Assessment
a. Referral Question: Let the client think about this. Maybe this individual has something he/she
wants to know about or a problem that needs to be addressed. Conduct an intake interview if
you prefer.
b. Select your assessment tools smartly. Administer tools that you think will fit the case. Aside from
your choice of tools, you are required to at least administer one projective test.
c. Request for a copy of a psychological test must be coursed through the professor of the
subject. These copies must be properly taken care of. If the test is not available, strategize how
to get the needed information using a different tool.
d. You must get the consent of the client for the psychological assessment. Have him/her sign one
before starting (This one you need to make).
e. Please do not tire your client. Divide the sessions of assessment. Don’t let him/her take all the
tests in one meeting.
f. Observe the right procedures in interviewing or talking to the client. Please apply your
counseling skills in communicating with them.
g. Make sure to score and interpret the tests correctly.
h. Take notes during interviews. Ask the consent first before recording conversations.
i. Organize all test protocols. Staple all these documents altogether, from answer sheets, to
scored tests, interpretation, and even interview notes. Score and interpret every test neatly and
clearly.
5. Case Conceptualization

a. Use the framework taught in class (Inverted Pyramid Model) to conceptualize the problem of the
client.
b. So, yes, submit a document showing how you used IPM to conceptualize the case.
6. Psychological Report
a. A sample is uploaded. Aim for the same quality as the sample.
b. Format and details will be discussed in class.
7. Treatment Plan
a. Must be clearly organized.
b. Elements of a successful treatment plan:
Behaviorally defining the presenting problems
Selecting Achievable goals
Determining Treatment Modes
Demonstrating Changes and Outcomes
8. How to Submit:
a. Print the Psych Report and Treatment Plan using 8x11 Letter Size Bond Papers
b. The Psychological Report and Treatment Plan must be stapled together.
c. Enclose the Psych Report and Treatment Plan in a short, colored, short folder. Use 15mm
binder clip to hold all the documents together (I think this is the smallest).
Each section will be assigned a color. Do not write anything on the folder.
d. Put the scored tests, notes, consent forms, IPM document, etc. in a short, brown envelope.
e. Put all inside a long, plastic, folder.
