Summary of Forms

A. Loan Forms
I. SSS Loan
Attachments:
a.) Photocopy of 1 Valid ID or 2 Secondary IDs with 3 specimen
signatures on attachment.

Steps:
1.) Print and fill-out form.
2.) Attach the necessary attachment/s
3.) Submit to HR for certification
4.) Await message from HR to pick-up signed form.

Notes:
*As of March 2019, SSS requires personal filing to apply for their cash
card (Unionbank).
*For members applying for the first time, for you to avail of the loan, it
is necessary that you have 36 monthly contributions to your SSS
* For those with previous loans, kindly check with your online SSS
account or call SSS at 920-6446 to 55 to verify if you are already
eligible to apply for a new loan.

II. PAGIBIG Loan


Attachments:
a.) Photocopy of 1 Valid ID or 2 Secondary IDs with 3 specimen
signatures on attachment.
b.) photocopy of latest 1 month payslip.

Steps:
1.) Print and fill-out form.
2.) Attach the necessary attachment/s
3.) Submit to HR for certification
4.) Await message from HR to pick-up signed form.

Notes:
*If you already have a valid cash card (those of DBP or Landbank with
EMV chips), please disregard step 4. The company representative will file the
form on your behalf.

III. CITIFIN Loan.


Attachments:
a.) Photocopy of 1 Valid ID or 2 Secondary IDs with 3 specimen
signatures on attachment.
b.) Photocopy of latest 1 month payslip Or BIR form 2316

Steps:
1.) Print and fill-out form.
2.) Attach the necessary attachment/s
3.) Submit to HR for certification.
4.) Await for CITIFIN to contact you.

B. Maternity, Sickness, and Hospitalization forms


IV. Firstlife reimbursement
Attachments:
a.) Medical certificate (original copy)
b.) Medicine prescriptions (original copy)
c.) Receipt of expenses incurred (original copy; e.g. medicines,
laboratory tests, etc.)

Steps:
1.) Print and fill-out form
2.) Attach necessary attachments
3.) Submit to Ms. Shane of HR.

Notes:
* All documents should be original copies.
* Reimbursement claims would take atleast 1 month after submission of
documents.

V. SSS Maternity Notification Form (Mat-1)


Attachments:
a.) 2 Valid IDs with 3 specimen signature on page.
b.) Hospital Documents (With expected date of delivery; Medical
Certificate; All original copies)
c.) Summary list of contributions for the preceding year.

Steps:
1.) Fill-up form
2.) Submit Attachments
3.) Await notification from HR 1 to 2 months before delivery for claiming
of cheque.

Notes:
* All hospital documents should be original copies
* Members should have at least three (3) monthly contributions within
the 12-month period immediately preceding the application.
* Claim for maternity benefit automatically disqualifies the member from
availing of sickness benefit for the same period

VI. SSS Maternity Reimbursement Form (Mat-2)


Attachments:
a.) 2 Valid IDs with 3 specimen signature on page
b.) Birth certificate of child (Original; Certified true copy of Civil
Registrar)
c.) Hospital documents (Medical certificate, etc.; Original copies)
d.) Summary list of contributions for the preceding year.
Steps:
1.) Fill-up form
2.) Submit with attachments
3.) Await notification from HR 1 to 2 months after submission for
claiming of cheque

Notes:
*All hospital documents should be original copies
*For Caesarean delivery: submit any document that indicates the type of
delivery (e.g. discharge summary, operative record).

VII-VIII. SSS Sickness Notification / Sickness Reimbursement Form


Attachments:
a.) 2 Valid IDs with 3 specimen signature on page
b.) Hospital documents (Medical certificate, etc.; Original copies)
c.) Summary list of contributions for the preceding year.

Steps:
1.) Print and fill-out forms
2.) Submit to attending physician for their signature on the form
3.) Submit to HR with required attachments

Notes:
* Applies to employees who have been confined/hospitalized for 4 days
or more
* Form should include the expected date of fit-to-work and the start date
of the hospitalization/confinement
* Sick Leave will be exhausted first

C. Updating of status on government agencies forms (change/update status)

Attachments:
a.) Relevant documents for the requested change (e.g. birth certificate;
marriage certificate)

Steps:
1.) Download and fill-up the form of the particular government agency
2.) Submit to Ms. Centeno with the required attachments.
