Present Illness: Streptococcus pyogenes

Symptoms: Rash on the body, strep throat, cough, runny nose


Type: Group A Streptococcus
Severity: Acute
Location: Swollen glands in neck
Duration: -amoxicillin (AMOXIL) 400 -trimethoprim-polymyxin
mg/5 mL suspension (POLYTRIM) 10,000 unit-1
Influencing factors: Mom had it mg/mL ophthalmic solution
History of present illness: mom was dxd
Current drugs: No present drugs
Allergies: No known Allergies
Smokes: No
Drinks: No
Previous treatment: N/A
Previous Med treatment: N/A

Diagnosis: She tested positive for strep throat.

2. Take extra care to rest and drink plenty of fluids. Call office if there is no improvement in 2
days
3. You can treat fever or discomfort with ibuprofen/tylenol per bottle instructions
