By signing below, I confirm that I have read, understand and agreed to the following terms and
condition for Execom internet services.

1. Execom rely on the information supplied on the customer information sheet provided and providing
false or incorrect information may result to delay or termination of the application.
2. Execom will charge me the corresponding fee based on the options I will choose below.

* the Radio antenna cost Php. 12,000 will be divided equally in 12 month Plus Monthly recurring fee, after 12 months; your
monthly recurring fee will be 6,000 or 10,000 respectively.
** the Radio antenna cost Php. 12,000 will be divided equally in 24 month Plus Monthly recurring fee, after 24 months;
your monthly recurring fee will be 6,000 or 10,000 respectively.
***Pay for Php. 12,000 for Radio antenna cost Plus Php. 5,000 Installation fee and straight Monthly recurring fee for 6,000
or 10,000 respectively

3. I hereby choose the Option __________________ mentioned above.


4. That execom does not have any control on its Internet Service Provider (ISP), does if the ISP is down,
execom’s services will be affected too.
5. That I am locked in for 12/24 months for this Option _____________ I choose.
6. That if after contract terminated I will return the equipments owned by execom, otherwise services
will continue and monthly recurring fees.
7. That I am responsible if the modem get lost or Destroyed, and will be charged Php. 12,000.00
8. That I will hold Execom free from any liabilities from my previous Telco companies.
