PETITION
COMES NOW THE PETITIONER, KATHLEEN KAYE M. LAURENTE, in
her own behalf, and unto this Honorable Court, most respectfully states that:
1. She is a Filipino, single, over thirty- two (32) years old and a resident of
Kingdom of Jesus Christ Compound, Phil-Japan Friendship Highway,
Sasa, Davao City, copies of the picture of the petitioner with signature at
the bottom part are hereto attached as Exhibits “A”, “A-1” and “A-2”;
2. She has satisfactorily completed the required course in law at Jose Maria
College, Davao City, Davao del Sur on March 30, 2020;
3. She was admitted to the Philippine Bar on June 1, 2021, and she is a
member of the Integrated Bar of the Philippines (IBP) Davao City Chapter
as evidenced by a copy of her Certification of Membership in the
Philippine Bar issued by the Office of the Bar Confidant, Supreme Court,
and the same is attached hereto as Annex “B”;
4. She is a new applicant desiring to be commissioned as a Notary Public
for and in the City of Davao;
5. She possesses all qualifications and none of the disqualifications as
required by law for the office of notary public;
6. She has never been convicted of a crime involving moral turpitude,
neither does she have any pending administrative case against her and
this fact is evidenced by a Certification from the Office of the Bar
Confidant, Supreme Court, and the same is attached hereto as Annex
“C”;;
7. To support her Petition, the Petitioner submits the following documents:
a. Annex “D”- Certificate of Good Moral Character from IBP – Davao
City Chapter;
b. Annex “E” - Impression of Notarial Seal;

c. Annex “F”- Photocopy of the Official Receipt from the
manufacturer of the Notarial Seal;
d. Annex “G”- Photocopy of the Official Receipt of Professional Tax
Receipt No. 10262018 issued on June 25, 2021;
e. Annex “H”- Photocopy of Official Receipt of IBP membership with
Receipt No. 10261986 issued on June 1, 2021;
WHEREFORE, it is most respectfully prayed from this Honorable
Court that herein petitioner be commissioned as a Notary Public for and in the
City of Davao.
Petitioner prays for such other remedies as this Honorable Court may
deem just and equitable in the premises.

1. That I am the petitioner in the above-entitled Petition;
2. That I have caused the preparation and filing of the foregoing petition;
3. That I have read the same and attest that all the allegations therein
are true and correct to the best of my personal knowledge and based
on authentic records in my possession;
4. That I have not heretofore commenced any other action or proceeding
involving the same issue before any court or tribunal or before the
Supreme Court or the Court of Appeals and to the best of my
knowledge, no such action or proceeding is pending or withdrawn
from the Supreme Court, the Court of Appeals or the different
divisions thereof or any other tribunal or agency.
5. That should I hereafter learn of a similar action or proceeding has
been filed or pending before the Supreme Court, the Court of Appeals
or different divisions thereof, or any other court, tribunal or agency, I
hereby undertake to promptly inform this court of that fact within five
(5) days therefrom.
